奥马哈系统为框架的个案管理模式在糖尿病药学服务中的应用价值研究

摘　　要：目的分析奥马哈系统为框架的个案管理模式在糖尿病药学服务中的应用价值。方法选择2015年12月～2016年12月深圳市罗湖区人民医院收治的304例糖尿病患者，按照数字表随机分组法分为对照组和干预组，各152例。对照组给予常规治疗，干预组在此基础上给予奥马哈系统为框架的个案管理模式。患者出院后进行为期6个月的随访，观察2组患者血糖控制情况及用药合理性、依从性、不良反应等情况。结果研究期间，4例患者因病故或不符合标准退出研究计划。干预组患者依从率为96.67%，高于对照组的82.67%（P＜0.05），血糖控制情况及疾病相关知识知晓率均优于对照组，比较差异有统计学意义（均P＜0.05）。结论奥马哈系统为框架的个案管理模式是一种适合我国国情的糖尿病药学服务模式，提高患者的治疗依从性，改善患者的用药合理性，促进患者血糖的控制。Objective To evaluate the Omaha system analysis framework for case management model of the value of pharmaceutical care in diabetes.Methods A total of304patients with diabetes mellitus were selected from December2015to December2016.The patients were divided into control group and intervention group according to the randomized grouping method.The control group was given routine treatment,and the intervention group gave the Omaha system as a case management model.The patients were followed up for6months after discharge,and the blood glucose control and medication rationality,compliance and adverse reactions were observed.Results During the study period,four patients withdrew from the study plan due to illness or non-compliance.The compliance rate was96.67%in the intervention group and higher than82.67%in the control group(P<0.05).The rate of blood glucose control and disease-related knowledge were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The Omaha system case management model for the framework is a kind of diabetes pharmaceutical care mode suitable for China's national conditions,to improve the therapeutic compliance of patients,improve the rationality of the patient's medication,promote the patient blood sugar control,used to be promoted.
