无创正压通气联合抗胆碱能药物治疗慢性阻塞性肺疾病急性加重期合并Ⅱ型呼吸衰竭的疗效观察

摘　　要：目的探讨无创正压通气、抗胆碱能药物联合应用于慢性阻塞性肺疾病急性加重期（AECOPD）合并Ⅱ型呼吸衰竭患者临床治疗效果。方法选取西安医学院第二附属医院2016年5月～2017年2月期间收治的88例AECOPD合并Ⅱ型呼吸衰竭患者作为本次研究对象，随机分为研究组和对照组，各44例。对照组在常规治疗基础上加用无创正压通气，研究组在常规治疗基础上加用无创正压通气和抗胆碱能药物。比较2组患者PaO2、pH、PaCO2等血气分析指标情况。结果治疗后研究组PaO2、pH高于对照组，PaCO2低于对照组，差异具有统计学意义（P＜0.05）。结论应用无创正压通气、抗胆碱能药物联合治疗AECOPD合并Ⅱ型呼吸衰竭可提高患者临床治疗效果，有利于保障其生活质量、生命安全。Objective To investigate the clinical efficacy of noninvasive positive pressure ventilation combined with anticholinergic drugs in the treatment of chronic obstructive pulmonary disease with acute exacerbation of typeⅡrespiratory failure.Methods88cases of AECOPD combined with typeⅡrespiratory failure treated in the Second Affiliated Hospital of Xi’an Medical University from May2016to February2017were selected as the research subjects.They were randomly divided into the study group and the control group,44cases in each group.The control group was treated with noninvasive positive pressure ventilation on the basis of conventional treatment.The study group was treated with noninvasive positive pressure ventilation and anticholinergic drugs on the basis of routine treatment.PaO2,pH,PaCO2and other blood gas analysis were compared in the two groups.Results After treatment,the study group PaO2,pH was higher than the control group,and the PaCO2was lower than the control group,the difference was statistically significant(P<0.05).Conclusion The combination of nasal intermittent positive pressure ventilation and anticholinergic drugs in the treatment of AECOPD combined withⅡrespiratory failure can significantly improve the clinical efficacy of patients,and is conducive to the protection of their quality of life and life safety.
