低剂量氯吡格雷对老年冠心病合并慢性肾病患者血小板抑制率及MACE的影响

摘　　要：目的探讨低剂量氯吡格雷对老年冠心病合并慢性肾病患者血小板抑制率及MACE的影响。方法选择2015年1月～2016年12月陕西省宝鸡市中心医院收治的59例老年冠心病合并慢性肾病患者，根据入院顺序分为观察组（31例）及对照组（28例），观察组给予50mg/d氯吡格雷治疗，对照组给予75mg/d氯吡格雷治疗，对比2组的治疗前、治疗3个月、6个月血小板抑制率，MACE发生率及不良反应发生率。结果治疗后6个月，2组的血小板抑制率及二磷酸腺苷抑制率均明显低于治疗前（P＜0.05）；观察组的不良反应发生率低于对照组（P＜0.05）。结论低剂量氯吡格雷治疗老年冠心病合并慢性肾病患者，对血小板抑制率及MACE发生率同常规剂量相当，但不良反应发生率较常规剂量低。Objective To investigate the influence of low dose clopidogrel on platelet inhibition ratio and MACE of patients with coronary heart disease combined with chronic kidney disease.Methods59case with coronary heart disease combined with chronic kidney disease from January2015to December2016in Baoji central hospital were chosen and divided into the observation group(31cases)and control group(28cases),the observation group were given50mg/d clopidogrel,the control group were given75mg/d clopidogrel.The platelet inhibition ratio of two groups before treatment,after treatment for three months,six months were compared,the MACE incidence rate and adverse reaction incidence rate of two groups were compared.Results After treatment for six months,the platelet inhibition ratio and adenosine diphosphate inhibition rate of two groups were lower than before treatment(P<0.05).The adverse reaction incidence rate of observation group were lower than control group(P<0.05).Conclusion Low dose clopidogrel in treatment of coronary heart disease combined with chronic kidney disease had equivalent effects on platelet inhibition ratio,and the MACE incidence rate was equivalent to conventional dose,while the adverse reaction of low dose clopidogrel was lower than conventional dose.
