双球囊扩张、欣普贝生及催产素促宫颈成熟的疗效和对宫颈BishoP评分的影响

摘　　要：目的探讨双球囊扩张、欣普贝生及催产素促宫颈成熟的疗效和对宫颈BishoP评分的影响。方法选取2016年2月～2017年2月浙江省嘉兴市妇幼保健院产科收治的足月妊娠初产妇84例，按随机数字表法均分为3组，缩宫素组28例采取催产素静滴的方式。欣普贝生组28例采取普贝生栓剂阴道内用药的方式。双球囊组28例采用双球囊扩张术。比较3组孕妇阴道分娩率、剖宫产分娩率、引产至临产时间、宫颈BishoP评分及新生儿Apgar评分等。结果与缩宫素组、欣普贝生组产妇比较，双球囊组宫颈BishoP评分升高最为明显，促宫颈成熟总有效率相较于其他2组明显提高，其次为欣普贝生组，3组差异有统计学意义（P＜0.05）。与缩宫素组、欣普贝生组产妇比较，双球囊组引产至临产时间缩短最为明显，阴道分娩率最高，剖宫产率最低，产妇并发症、新生儿宫内窘迫发生率都明显最低，差异有统计学意义（P＜0.05）。欣普贝生组、双球囊组、缩宫素组3组新生儿APgar评分及新生儿体质量比较差异无统计学意义。结论采取双球囊扩张的手术方式可有效促进宫颈成熟，具有良好的软化宫颈作用，对产妇的睡眠质量、生活质量、分娩结局都有良好作用。Objective To investigate the curative effect of double balloon dilatation,dinoprostoneand oxytocin on cervical ripening and its effect on cervical BishoP score.Methods84primiparae of full-term pregnancy admitted to obstetrics and gynecology from February2016to February2017were divided into three groups according to the random number table method.28cases of oxytocin group were treated with intravenous oxytocin.28cases in the dinoprostone group were treated with dinoprostone suppositories intravaginally.28cases of double balloon group were given double balloon dilatation.Vaginal delivery rate,cesarean section delivery rate,induction of labor to labor time,cervical BishoPscore and neonatal Apgar score were compared among in the three groups.Results Compared with oxytocin group and dinoprostone group,the cervical BishoP score of the double balloon group increased most obviously.The total effective rate of promoting cervical ripening in the double balloon group was significantly higher than that of the other two groups,followed by dinoprostone group,the difference in the three groups was statistically significant(P<0.05).Compared with the oxytocin group and the dinoprostone group,the double balloon group had the most obvious induction of labor to shorten the time of labor,with the highest vaginal delivery rate,the lowest cesarean section rate,the lowest maternal complications and neonatal intrauterine distress rate,the difference was statistically significant(P<0.05).There were no significant differences in APgar score and fetal weight among dinoprostonegroup,double balloon group and oxytocin group.Conclusion The operation of double balloon dilation can effectively promote the cervical ripening,has a good softening cervix and has a good effect on maternal sleep quality,quality of life and delivery outcome.
