摘  要:目的建立大鼠心肌梗死后心力衰竭动物模型早期评价和筛选新方法,为药物干预研究的早期随机分组提供依据。方法SD大鼠随机分为模型组和假手术组。结扎大鼠心脏左冠状动脉前降支建立心肌梗死模型,假手术组在相同位点,只穿线不结扎。两组分别于术后第2天、第10天和第60天进行12导联心电图(ECG)检测;术后第10天和60天,采用小动物超声仪进行心功能检测。分别对不同时间点的心电图病理性Q波与左室射血分数(EF)和左室短轴缩短率(FS)进行相关性分析。结果模型组大鼠术后第2天、第10天以及第60天心电图病理性Q波个数与超声心动图EF和FS值存在显著的负相关性。若EF<50%,病理性Q波分布于V3~V6、Ⅰ、aVL兼有V1或V2;若EF<30%,除上述导联外,由于镜像效应出现下壁Ⅱ、Ⅲ、aVF中单一或多个导联的病理性Q波。应用第2天ECG病理性Q波进行心肌梗死后心衰动物模型早期筛选的特异性为100.0%,敏感性为95.0%,准确性为96.7%。结论心肌梗死后心力衰竭动物模型不同时间点的ECG病理性Q波个数与EF和FS值存在显著的负相关性,可用于动物模型的早期评价和筛选。此方法操作简单易行,效果理想,为药理实验的早期评价和随机分组提供了重要和可靠的实验依据。Objective To establish a new method for early evaluation and screening of heart failure(HF)model after myocardial infarction(MI)and provide basis for early random grouping on drug intervention study.Methods Sprague Dawley(SD)rats wererandomly divided into model and sham group.The rats in MI models were established by ligating left anterior descending coronary artery,while sham rats underwent thoracotomy without ligation.The12lead electrocardiogram(ECG)were detected for the twogroups on2nd,10th and60th day after operation.On the10th and60th post operative day,the ultrasonic cardiogram were detected.Then the correlation between the different time point’s pathological Q wave and ejection fraction(EF)and fractional shortening(FS)was analyzed.Results There was a significant negative correlation between the number of different time point’s pathological Q waveand EF and FS index.Pathological Q waves were distributed in V3~V6,Ⅰand aVL leads,with both V1and/or V2leads if the EF<50%.When the EF<30%,besides the above leads,it also extended to the single or multiple inferior wall leads ofⅡ,Ⅲ,aVF due tothe mirror effect.The specificity,sensitivity and accuracy of early screening of animal models with2nd day pathological Q wave were100.0%,95.0%and96.7%.Conclusion Different time point’s pathological Q wave had significant negative correlation with EF andFS,which could be used for early screening of HF after myocardial infarction models.The study provides reliable experimental evidencefor early evaluation and randomization of pharmacological experiments.
