血清HCG定量在宫外孕早期诊断及临床治疗中的应用

摘　　要：目的:探讨血清HCG定量在宫外孕的早期诊断及治疗中的应用价值。方法:收治宫外孕患者127例,同期收治流产14例,先兆流产35例,难免流产17例。比较血清HCG水平。结果:异位妊娠孕囊未破裂者、孕囊破裂者血清HCG与孕周存在相关性。手术治疗91例患者术后24 h复查血清HCG全部转阴,保守治疗36例患者,成功者治疗后第1个周期血HCG低于治疗前(P<0.05)。失败者治疗后第1个周期血HCG低于治疗后,但高于成功者(P<0.05)。失败者一过性血HCG上升100.0%,高于成功者29.2%(P<0.05)。结论:血清HCG定量分析可用于鉴别诊断异位妊娠,同时预测疗效,分析保守治疗的成功率,判断是否存在子宫残留。Objective:To investigate the value of serum HCG quantification in the early diagnosis and treatment of ectopic pregnancy.Methods:127cases of ectopic pregnancy were selected,14cases of abortion,35cases of threatened abortion,17cases of inevitable abortion were selected in the same period.Results:There was correlation between serum HCG and gestational age in unruptured ectopic pregnancy and rupture of gestational sac.After operation,the serum HCG of all the91patients were negative after24h,and36patients were treated conservatively.The blood HCG of the patients in the first cycles after treatment was lower than that before the treatment(P<0.05).The blood HCG in the first cycles after failure was lower than that in the treatment group,but higher than that in the successful group(P<0.05).The transient blood HCG of the failure increased by100%,higher than that of the successful group29.2%(P<0.05).Conclusion:Serum HCG quantitative analysis can be used for differential diagnosis of ectopic pregnancy,while predicting the efficacy,analyze the success rate of conservative treatment,to determine whether there is residual uterus.
