摘  要:目的:分析妊娠期宫内窘迫胎儿大脑中动脉(MCD)及肾动脉血流动力学指标,为临床诊断提供依据。方法:回顾分析160例分娩新生儿,根据产后新生儿有无宫内窘迫将其分为宫内窘迫组和正常组,每组80例。采用彩色多普勒血流成像(CDFI)监测胎儿MCD及肾动脉各项血流指标,观察比较两组胎儿MCD、肾动脉搏动指数(PI)、阻力指数(RI)和血流收缩期峰值(S)与舒张期峰值(D)的比值(S/D)的血流力学指标。结果:宫内窘迫组与正常妊娠胎儿相比,胎儿MCD血流力学指标PI、RI和S/D值分别为(2.51±0.15)、(0.91±0.09)和(5.62±0.82),均低于正常组的(1.56±0.19)、(0.78±0.08)和(4.16±0.42),其差异均有显著统计学意义(t=4.52,t=4.33,t=4.31;P<0.01);宫内窘迫组胎儿肾动脉的平均PI、RI和S/D值为(2.51±0.15)、(0.91±0.09)和(5.62±0.82),均高于正常组胎儿的(1.66±0.21)、(0.77±0.06)和(3.86±0.25),其差异均有显著统计学意义(t=4.23,t=4.36,t=4.22;P<0.01)。结论:采用彩色多普勒超声检测胎儿MCD、肾动脉血流力学指标评价胎儿是否宫内窘迫具有重要意义,值得临床推广使用。Objective:To analyze the hemodynamic indexes of middle cerebral artery(MCD)and renal artery of fetal distress during pregnancy so as to provide basis for clinical diagnosis.Methods:160puerperal newborns were researched by using retrospective analysis,and they were divided into intrauterine distress group(80cases)and normal group(80cases)depended on whether happened postpartum neonatal intrauterine distress.Color Doppler flow imaging(CDFI)was used to monitor fetal MCD and blood flow indexes of renal artery,and series of hemodynamic indexes included of MCD,pulsatility index(PI)and resistance index(RI)of renal artery and the ratio of peak systolic flow(S)and diastolic peak(D)(S/D)of the two groups were observed and compared.Results:The PI,RI and S/D of intrauterine distress group(2.51±0.15),(0.91±0.09)and(5.62±0.82)were significantly lower than those of normal group(1.56±0.19),(0.78±0.08)and(4.16±0.42),respectively(t=4.52,t=4.33,t=4.31,P<0.01).The means of PI,RI and S/D values of fetal renal artery in intrauterine distress group(2.51±0.15),(0.91±0.09)and(5.62±0.82)were significantly higher than those in the normal group(1.66±0.21),(0.77±0.06)and(3.86±0.25),respectively(t=4.23,t=4.36,t=4.22,P<0.01).Conclusion:Using color Doppler ultrasonic to detect fetal MCD and hemodynamic parameters of renal artery has important significance for evaluating whether happens fetal distress.Therefore,it is worthy in clinical application.
