摘 要:目的动脉波速指标(AVI)和动脉压力容积指标(API)是由动脉脉搏波速率检测仪(AVE-1500型)经波形解析无创估测的动脉硬化指标,分别指示中心动脉和肱动脉的僵硬度,其临床价值已在国外得到了广泛验证,但在我国尚无相关报道。本文旨在通过研究AVI、API与动脉硬化的独立预测因子年龄和动脉僵硬度经典指标baPWV(肱踝脉搏波传导速度)的相关性,评估其作为动脉僵硬度指标的有效性,从而为该检测方法的临床应用提供初步依据。方法在体检中心随机募集受试者,对每位受试者分别采用动脉脉搏波速率检测仪测量AVI、API以及上臂动脉血压,其后使用动脉硬化测量仪(BP-203RPEⅢ型)测量受试者的双侧baPWV。同时,对所有受试者均记录年龄、性别和血液生化指标等一般临床信息。应用Pearson相关分析探讨AVI、API与年龄及ba PWV的相关性;采用检验研究高血压患者组与年龄、性别匹配的健康组测量结果的差异性。结果共募集140例受试者,包括110例健康成年人(47.4±14.4)岁和30例单纯高血压患者(53.5±10.6)岁。结果显示,健康受试者组,AVI与年龄呈高度线性相关(r=0.706,P=0.000),且与baPWV呈显著线性相关(r=0.599,P=0.000),API与年龄、baPWV亦相关,但相关程度明显低于AVI。30例高血压患者与年龄、性别匹配的30例健康受试者比较,高血压组的AVI、API、baPWV大于健康组,其差异具有统计学意义。结论 AVI在监测由年龄、高血压引起的动脉硬化进展方面与baPWV具有相似的效果,API由于局限于对外周动脉(肱动脉)僵硬度的评估,在健康人群中与年龄的相关性较弱,但在评估心血管疾病患者的动脉弹性状况方面仍有一定价值。Objective To explore the correlation of arterial velocity pulse index(AVI)and arterial pressure volume index(API)to age and brachial-ankle pulse wave velocity(baPWV),the independent predictors of arterial stiffening,and to evaluate their validity as arterial stiffness indices.Methods Volunteers were randomly recruited from the physical examination center,in total,140volunteers were enrolled,including110healthy subjects(aged at47.40±14.38years)and30subjects with pure hypertension(aged at53.50±10.60years);arterial pulse velocity detector and arterial stiffness device(BP-203RPEIII)were applied in detecting AVI,API,left brachial blood pressures(BPs)and bilateral baPWVF of the volunteers;the clinical data of the volunteers including age,sex and blood biochemical index were recorded;Pearson Correlation Analysis was applied in exploreing the correlation of AVI and API to age and baPWV while t test was applied in detecting the difference in the explored data between the subjects with hypertension and the norm of same age and gender.Results AVI of the subjects in healthy group was in strong linear correlation with age(r=0.706,P=0.000)and was obviously correlated to baPWV(r=0.599,P=0.000)and API was also correlated to age and baPWV but was in a much weaker correlation than that AVI was in;the mean of AVI,API and baPWV of the subjects with hypertension were higher than those of the norm of same age and gender with a statistical difference.Conclusions AVI is of similar role with baPWV in detecting the arterial stiffening caused by age and hypertension;API is of certain value in evaluating the arterial elasticity in patients with hypertension though it has a weaker correlation with age in the healthy due to its limitation as a a stiffness index of peripheral(i.e.,brachial)artery.
