脊柱骨转移瘤在全身磁共振弥散加权成像中阴性显示原因分析

摘　　要：目的:分析脊柱骨转移瘤在全身磁共振弥散加权成像中阴性显示原因,以提高全身磁共振弥散加权成像诊断的准确性及减少脊柱骨转移瘤的漏诊。方法:回顾性分析12例在全身磁共振弥散加权成像中阴性显示的脊柱骨转移瘤病例,对脊柱骨转移瘤全身磁共振弥散加权图像、常规核磁共振图像、SPECT图像、CT图像进行分析。12例均行全身磁共振弥散加权成像、常规磁共振成像,5例同时行SPECT检查,4例同时行CT检查。结果:2例为成骨性转移,9例为溶骨性转移治疗后转归,1例为混合性骨转移瘤治疗后转归。12例全身磁共振弥散加权成像均呈阴性显示,12例常规磁共振图像均有低信号为主多发脊柱骨转移瘤显示,5例SPECT图像中有2例脊柱骨转移瘤显示,4例CT图像均呈多发高密度灶。结论:全身磁共振弥散加权成像中阴性显示病例需要密切结合常规核磁共振图像、SPECT图像、CT图像,以提供对脊柱骨转移的准确评价。Objective:To improve the accuracy diagnosis of whole-body-diffusion-weighted imaging and reduce the misdiagnosis of metastasis spine tumors,we analyze the causes of negative result of metastasis spine tumor in whole-body-diffusion-weighted imaging.Methods:12cases took a retrospective analysis,including the magnetic resonance imaging images of the spine bone metastases,conventional magnetic resonance images,SPECT images,CT images.12cases took whole-body-diffusion-weighted images and conventional MRI images,5cases with SPECT images,4cases with CT images.Results:According to the pathologic result,2cases are osteoblastic metastasis;9cases are osteolytic metastasis;1case is mixed bone metastases after treatment.12cases have negative result in whole-body-diffusion-weighted images,also have low signal in conventional MRI images.2cases of SPECT images show bone metastases;4cases of CT scans have multiple lesions with high density.Conclusion:To improve the accurate evaluation,patients with negative result in whole-body-diffusion-weighted images should be examined with the conventional MRI,SPECT and CT images.
