摘 要:目的评价经桡动脉路径行选择性冠状动脉造影的同时行选择性肾动脉造影的可行性、安全性及其临床意义。方法选择2016年3月~12月于湖北省十堰市人民医院住院伴有血压升高的疑似冠状动脉粥样硬化性心脏病(冠心病)患者52例,按不同的动脉路径分为两组:经桡动脉造影组28例,经股动脉造影组24例,均在选择性冠状动脉造影结束后即刻同路径行选择性肾动脉造影,比较两组肾动脉造影期间X线光照时间、手术操作时间、手术成功率及并发症发生率。结果两组共检出肾动脉狭窄8例,肾动脉狭窄总发生率为15.4%。两组的X线光照时间、手术操作时间无显著性差异(P>0.05);经桡动脉造影组的手术成功率明显低于经股动脉造影组(85.71%vs 95.83%),差异有统计学意义(P<0.05);经桡动脉造影组的血管并发症发生率明显低于经股动脉造影组(3.57%vs 12.50%),差异有统计学意义(P<0.05)。结论在目前经桡动脉行选择性冠状动脉造影大量开展的情况下,经同路径行选择性肾动脉造影以明确肾动脉狭窄程度,具有安全、可行、并发症少的优点,并可通过改进导管长度提高手术成功率。Objective To review the feasibility,safety and clinical significance of implementing selective renal angiography at the same time of implementing selective coronary angiography via radial artery approach.Methods The patients(n=52)with increased blood pressure and suspected coronary heart disease(CHD)were chosen from the People’s Hospital of Shiyan City of Hubei Province from Mar.2016to Dec.2016.All patients were divided,according to different artery approaches,into radial angiography group(n=28)and femoral angiography group(n=24),and given selective renal angiography immediately after selective coronary angiography via corresponding approaches respectively.The time of X-ray exposure,operation duration,operation success rate and incidence of complications were compared between2groups.Results There were8cases of renal artery stenosis detected from2groups,and the total incidence of renal artery stenosis was15.4%.The time of X-ray exposure and operation duration had no significant difference between2groups(P>0.05).The operation success rate was significantly lower in radial angiography group than that in femoral angiography group(85.71%vs.95.83%,P<0.05).The incidence of vascular complications was significantly lower in radial angiography group than that in femoral angiography group(3.57%vs.12.50%,P<0.05).Conclusion With the popularity of implementing selective coronary angiography via radial artery approach,implementing selective renal angiography via the same approach for clearing severity of renal artery stenosis is safe and feasible with less complications.The operation success rate can be improved through increasing catheter length.
