厄多司坦联合大环内酯类药物及鼻用糖皮质激素治疗慢性鼻-鼻窦炎伴鼻息肉的临床疗效观察

摘　　要：目的评估厄多司坦联合大环内酯类药物及糖皮质激素治疗慢性鼻-鼻窦炎伴鼻息肉(CRSw NP)的临床疗效及安全性。方法采用双盲、随机对照试验,选取2014年1月至2016年11月该院收治的CRSw NP患者80例,随机分为对照组和研究组,每组40例。对照组给予局部鼻喷糖皮质激素联合大环内酯类药物及安慰剂空胶囊口服;观察组给予局部鼻喷糖皮质激素联合大环内酯类药物及厄多司坦口服。采用疼痛视觉模拟评分(VAS)、Lund-Kennedy评分法、鼻腔鼻窦结局测试(SNOT-20)量表评分评价疗效。结果两组患者治疗后鼻塞、前和(或)后鼻漏VAS均明显下降,且研究组鼻塞、前和(或)后鼻漏VAS明显低于对照组,差异均有统计学意义(P<0.05)。两组患者鼻塞症状均不同程度缓解,前和(或)后鼻漏VAS评分,Lund-Kennedy评分和SNOT-20量表评分均明显下降(P<0.05),且研究组前和(或)后鼻漏VAS评分,Lund-Kennedy评分和SNOT-20量表评分较对照组显著下降(P<0.05)。研究组未见明显不良反应。结论厄多司坦联合大环内酯类药物及鼻用糖皮质激素治疗CRSw NP的临床疗效显著,耐受性好。ObjectiveTo evaluate the effect and safety of erdosteine combined with macrolide and intranasal corticosteroids in the treatment of chronic rhinosinusitis with nasal polyposis(CRSwNP).MethodsA double-blind randomized clinical trial was conducted,80patients with CRSwNP were enrolled and randomly divided into the control group and the experimental group,40cases in each group.The control group was given local intranasal corticosteroids combined with macrolide and oral placebo,while the experimental group were given local intranasal corticosteroids combined with macrolide and oral erdosteine.All patients were treated for3months.Visual analogue scale(VAS),Lund-Kennedy score and sino-nasal outcome test-20scales(SNOT-20)were employed to evaluate the clinical efficacy.ResultsAfter3months of medication,the scales of VAS including nasal blockage,anterior or posterior rhinorrhea,Lund-Kennedy score and SNOT-20score in both groups were improved more obviously than those before medication with statistical significance(P<0.05).The VAS of anterior or posterior rhinorrhea,anterior or posterior rhinorrhea in the experimental group reduced more obviously than those in the control group with statistical significance(P<0.05).No adverse effects appeared in the experimental group.ConclusionThe combination of erdosteine and intranasal corticosteroids has obviously clinical effects on treating CRSwNP with good tolerance.
