摘 要:目的观察白内障超声乳化吸除联合人工晶状体植入术对眼底黄斑中心凹视网膜和脉络膜厚度的影响。方法选取2014年1月至2016年1月在郑州市第二人民医院眼科行白内障超声乳化吸除联合人工晶状体植入术治疗的白内障患者62例(101眼),记录并比较患者术前1 d,术后1 d,1周,1、3、6个月光学相干断层成像仪检测眼底黄斑中心凹视网膜和脉络膜厚度,分析术前黄斑中心凹视网膜和脉络膜厚度与患者年龄、眼轴长度的相关性及术后脉络膜厚度变化与有关指标的相关性。结果术前黄斑中心凹视网膜与患者年龄、眼轴长度无关(r=0.061、0.046,P>0.05);术前黄斑中心凹脉络膜厚度患者与年龄、眼轴长度呈负相关(r=-0.821、-0.912,P<0.05);术后1 d,1周,1、3、6个月黄斑中心凹视网膜厚度分别为(225.9±22.6)、(226.6±21.9)、(232.4±19.5)、(227.0±19.4)、(225.8±17.2)μm,术后1个月视网膜厚度明显高于术前,差异有统计学意义(P<0.05);术后1 d,1周,1、3、6个月黄斑中心凹脉络膜厚度分别为(263.8±52.1)、(267.5±49.9)、(270.4±51.7)、(269.7±53.2)、(260.5±47.8)μm,术后1周,1、3个月脉络膜厚度明显高于术前,差异均有统计学意义(P<0.05)。术后1、3个月脉络膜厚度改变与眼轴长度呈负相关(r=-0.812、-0.805,P<0.05),与术前脉络膜厚度呈正相关(r=0.751、0.792,P<0.05),但与患者年龄、术前视网膜厚度、视网膜厚度改变均无关(P>0.05)。结论黄斑中心凹脉络膜厚度与患者年龄、眼轴长度呈负相关;在白内障超声乳化吸除联合人工晶状体植入术后,黄斑中心凹视网膜和脉络膜厚度均增加,脉络膜厚度增加持续时间更长,厚度增加更为显著。脉络膜厚度变化与眼轴长度呈负相关,与术前脉络膜厚度呈正相关。Objective〓To observe the effect of cataract phacoemulsification combined with intraocular lens implantation on retina and choroidal thickness in fundus macular central fovea.Methods〓Sixty?鄄two cataract patients(101eyes)with cataract phacoemulsification combined with intraocular lens implantationn in the Zhengzhou Municipal Second People′s Hospital from January2014to January2016were selected.The retinal thickness of fundus macular central fovea and choroidal thickness under macular central fovea by the optical coherence tomography were recorded and compared among preoperative1d,postoperative1d,postoperative1week,postoperative1,3,6months.The correlation between the retinal thickness and choroidal thickness with the age and optical axial as well as the correlation between postoperative choroidal thickness change and related indexes were analyzed.Results〓The preoperative retinal thickness had no relationship with the age and optical axial length of the patients(r=0.061,0.046,P>0.05);the preoperative choroidal thickness under macular central fovea was negatively correlated with the age and the optical axial length of the patients(r=-0.821,-0.912,P<0.05);the retinal thickness of macular central fovea on postoperative1d,1week,1,3,6months were(225.9±22.6),(226.6±21.9),(232.4±19.5),(227.0±19.4),(225.8±17.2)μm respectively,in which the retinal thickness at postoperative1month was significantly higher than that before operation,the difference was statistically significant(P<0.05);the choroidal thickness on postoperative1d,at postoperative1week,1,3,6months were(263.8±52.1),(267.5±49.9),(270.4±51.7),(269.7±53.2),(260.5±47.8)μm respectively,in which the choroidal thickness at postoperative1week,1,3months were significantly higher than that before operation,the difference was statistically significant(P<0.05).The choroidal thickening change at postoperative1,3months had a negative correlation with the optical axial length(r=-0.812,-0.805,P<0.05)and a positive correlation with preoperational choroidal thick
