摘 要:目的通过对同期流行性感冒(简称流感)样病例(ILI)监测结果的对比,分析ILI核酸检测阳性率波动原因,确保网络实验室核酸检测结果的准确、可靠,及时预防流感暴发疫情的发生。方法依据中国流感监测信息系统,选取重庆市永川区2014—2017年第1~10周检测数据进行对比分析,并随机抽取2017年送检的ILI鼻咽拭子样本和以往核酸检测阳性鼻咽拭子原始标本,分别用硕世生物科技有限公司生产的核酸提取试剂盒和全自动核酸提取仪、上海之江生物科技股份有限公司生产的核酸提取试剂盒和EX2400全自动核酸提取仪、手动核酸提取3种方法提取核酸,使用实时荧光定量PCR仪检测并对结果进行对比分析。结果2014—2017年第1~10周ILI比例分别为0.70%、0.67%、0.53%、0.40%,同期ILI比较,差异有统计学意义(χ2=56.556,P<0.05)。0~<5岁组、5~<15组不同年龄ILI比例比较,差异有统计学意义(χ~2=27.753、13.480,P<0.05)。各年度第1~10周核酸检测阳性率差异较大,2017年出现反常现象。采用3种不同核酸提取方法复检,检测结果与重庆市永川区流感网络实验室首次检测后在"中国流感监测信息系统"中报告的结果相符,没有出现错报和漏报情况。结论作为流感网络实验室人员,发现阳性率波动或反常时应高度重视,分析原因,并与各年度同期实验对比,推测流行趋势。Objective〓To simply analyze the reasons of positive rate fluctuation in influenza?鄄like illness(ILI)nucleic acid detection by the contrast of ILI monitoring results in the same period in order to ensure the accuracy and reliability of network laboratory nucleic acid detection results and timely prevent the occurrence of flu outbreak.Methods〓According to the National Influenza Surveillance Information System,the annual1-10weeks testing data of influenza in the Yongchuan District of Chongqing City during2014-2017were collected for conducting the contrastive analysis.The nasopharyngeal swab samples of flu cases and previous positive samples of nucleic acid detection were randomly extracted.Then nucleic acid was extracted by using the nucleic acid extracting reagent kits from the Shuoshi Biotechnology Co.,Ltd and automatic nucleic acids extraction apparatus,nucleic acid extracting reagent kits from the Shanghai Zhijiang Biotechnology Co.,Ltd and EX2400automatic nucleic acids extraction apparatus,and the manual nucleic acid extraction.Real?鄄time fluorescent PCR was used to conduct the comparative analysis on the detection results.Results〓The ILI proportions in the first10weeks during2014?鄄2017were0.70%,0.67%,0.53%and0.40%respectively,the difference was statistically significant compared with ILI at the same period(χ2=56.556,P<0.05).The ILI proportion had statistically significant difference between the0-<5years old group and5-<15years old group(χ2=27.753,13.480,P<0.05).The positive rate of nucleic acid detection at the first10weeks of each year had large difference among different years.The abnormal phenomenon occurred in2017.The re?鄄detection was performed by adopting3different nucleic acid extraction methods.The detection results were coincident with that in the National Influenza Surveillance Information System reported by the flu network laboratory of Yongchuan District of Chongqing City after the first time detection.No mistake report and missed report situation was coincident.Conclusion〓When th
