摘　　要：目的探讨肾康注射液与金水宝胶囊联合用药对冠心病患者经皮冠状动脉介入治疗(PCI)术后造影剂肾病的预防作用。方法常规治疗组(A组,52例)、肾康注射液组(B组,50例)和肾康注射液联合金水宝胶囊组(C组,50例)。A组常规给予降脂、抗血小板聚集、水化等治疗;B组在给予常规治疗的同时于PCI术前当天及术后72 h每日给予肾康注射液40 m L+5%葡萄糖液250 m L静脉滴注1次/天。C组除给予常规治疗外与PCI术前当天及术后3 d连续给予肾康注射液40 m L+5%葡萄糖液250 m L静脉滴注1次/天联合金水宝胶囊,口服3粒,3次/天。检测三组患者PCI术前及术后72 h血肌酐(Cr)、尿素氮(BUN)和肾小球滤过率(GFR),观察对比剂肾病(CIN)的发生率。结果 Cr、BUN及GFR等三项指标,术前三组比较均差异无统计学意义(P>0.05)。术后比较,各项指标均差异有统计学意义(P<0.017)。此外发生CIN者A组11例、B组3例、C组1例。三组比较差异有统计学意义(P<0.017)。结论肾康注射液联合金水宝胶囊对冠心病患者PCI术后CIN的发生具有一定的预防作用,其疗效优于单纯应用肾康注射液。Objective To observethe preventive effect of Shenkang Injection combined with Jinshuibao Capsule on contrast induced nephropathy in patients with percutaneous coronary intervention(PCI).MethodsA hundred and fifty two patients with coronary disease who were treated with PCI were randomlyassigned into three groups:group A was general treatment group(52patients),who were treated with conventional therapy including declining fat,anti platelet aggregation and hydration therapy;group B had50patients who,in addition to the conventional therapy,were injected with shenkang injection(40mL shenkang injection+5%glucose liquid,iv,1time/day)in preoperative day and every72h after operation;group C also had50patients,who were not only treated with the common treatment but also used shenkang injection(40mL shenkang injection+5%glucose liquid,iv,1time/day)combined with Jinshuibao Capsule(3particles/time,oral,3times/day)in preoperative day and every72h after operation.Three indicators including serum creatinine(Cr),blood urea nitrogen(BUN),glomerular filtration rate(GFR)were detected in preoperative day and72h after operation,in addition,the incidence rate of contrast induced nephropathy(CIN)was observed.ResultsThere were no significant differences in Cr,BUN and GFR among three groups before operation(P>0.05),yet there were significant differences in all those indicators among three groups after operation(P<0.017).What′s more,the number of CIN patients in different group was11(group A),3(group B)and1(group C),respectively.There were significant differences among three groups(P<0.017).ConclusionsThe treatment of shenkang injection combined with Jinshuibao Capsule has preventive effect on contrast induced nephropathy in patients with PCI,which had more curative effect than single use ofshenkang injection.
