利多卡因湿敷联合酮铬酸丁三醇在痔疮手术后镇痛的护理效果

摘　　要：目的探讨利多卡因湿敷联合酮铬酸丁三醇在痔疮术后镇痛的护理效果以及患者的满意度。方法选取2016年9月~2017年6月期间收治的肛肠外科患者111例,依据是否实施利多卡因湿敷减轻疼痛等护理措施进行临床分组,分为对照组(55例)和利多组(56例),研究两组患者首次排便的FPS-R评分、术后首次换药FPS-R评分、镇痛药剂量、满意度评分及住院时间。结果对照组与利多组患者首次排便的FPS-R评分[(8.60±1.15)vs(3.83±1.79)分,P=0.025]、术后首次换药FPS-R评分[(7.42±0.98)vs(3.48±1.02)分,P=0.000],差异均有统计学意义(P<0.05);利多组患者镇痛针剂药使用剂量明显少于常规组[(90.10±12.56)vs(180.31±10.46)mg,P=0.002]、利多组患者满意度评分明显高于对照组[(5.66±1.46)vs(2.13±0.91)分,P=0.016](P<0.05)。结论痔疮患者术后给予利多卡因湿敷联合酮铬酸丁三醇能够缓解患者的术后疼痛,减少镇痛针剂药剂量,提高患者住院满意度。Objective To investigate the nursing effect of lidocaine wet compress combined with ketorolac tromethamine on analgesia after hemorrhoid surgery and the satisfaction of patients.Methods A total of111patients with anorectal surgery admitted from September2016to June2017were clinically divided into control group(n=55)and lidocaine group(n=56)according to whether or not nursing measures such as lidocaine wet compress were used to reduce the pain.The first defecation FPS-R score,the first postoperative dressing FPS-R score,the analgesic dose,the satisfaction score,and the hospitalization time in the two groups were studied.Results There were significant differences in FPS-R scores of the first defecation[(8.6±1.15)vs(3.83±1.79)]and the first dressing[(7.42±0.98)vs(3.48±1.02)]between the control group and the lidocaine group(P<0.05).The dose of analgesic injection was significantly lower in the lidocaine group[(90.10±12.56)mg]than that in the control group[(180.31±10.46)mg](P=0.002).The scores of satisfaction in the lidocaine group[(5.66±1.46)]were significantly higher than those of the control group[(2.13±0.91)](P<0.05).Conclusion The administration of lidocaine combined with ketorolac tromethamine in patients with hemorrhoids after surgery can relieve the postoperative pain,reduce the dose of analgesic injection and improve the hospitalization satisfaction.
