摘  要:目的观察盐酸右美托咪定(Dex)预处理对高糖状态下大鼠肾缺血-再灌注后肾功能的影响。方法将36只成年雄性SD大鼠随机均分为6组:假手术组(NG-Sham组)、缺血-再灌注组(NG-I/R组)、盐酸右美托咪定预处理组(NG-Dex组)、高糖假手术组(HG-Sham组)、高糖缺血-再灌注组(HG-I/R组)、高糖盐酸右美托咪定预处理组(HG-Dex组)。除NG-Sham组和HG-Sham组外,其他各组制备肾缺血-再灌注模型,NG-Dex组、HG-Dex组于缺血前30 min腹腔注射Dex 50μg·kg^(-1),HG-Sham组、HG-I/R组、HG-Dex组腹腔注射25%葡萄糖溶液3 g·kg^(-1)后进行上述操作。测定各组血糖、肾功能,苏木精-伊红(HE)染色观察肾组织形态学变化,原位末端转移酶标记(TUNEL)法测定细胞凋亡情况。结果NG-I/R组血尿素氮(BUN)、血肌酐(Cr)、细胞凋亡率均高于NG-Sham组(P<0.05);NG-Dex组BUN、Cr、细胞凋亡率均低于NG-I/R组(P<0.05);HG-I/R组、HG-Dex组BUN、Cr、细胞凋亡率均分别高于NG-I/R组、NG-Dex组(P<0.05),HG-I/R组与HG-Dex组间差异无统计学意义(P>0.05)。结论盐酸右美托咪定对肾缺血-再灌注损伤后肾功能具有保护作用,该保护作用在高糖环境中被抑制,可能与高糖状态下肾脏细胞凋亡增多有关。ObjectiveTo investigate the effect of dexmedetomidine hydrochloride(Dex)preconditioning on renal function in rats after renal ischemia reperfusion injury under high glucose condition.MethodsSD rats were randomly divided into6groups:NG Sham operated group,NG I/R group,NG Dex group,HG Sham operated group,HG I/R group,HG Dex group.Renal ischemia reperfusion model was established except Sham groups.Dex50μg·kg-1was injected intraperitoneally30min before ischemia in the Dex preconditioning group,25%glucose3g·kg-1was given intraperitoneally before the renal ischemia reperfusion model was established in high glucose groups.Blood glucose and renal function of each group were detected.Renal pathologic changes were observed with hematoxylin eosin staining.Apoptosis of renal tissue was detected by TUNEL method.ResultsBUN,Cr and apoptosis rate in NG I/R group were higher than those in NG Sham operated group(P<0.05);BUN,Cr and apoptosis rate in NG Dex group were lower than those in NG I/R group(P<0.05);BUN,Cr and apoptosis rate in HG I/R group and HG Dex group were higher than those in NG I/R group and NG Dex group,respectively(P<0.05);However,there was no significant difference between HG I/R group and HG Dex group(P>0.05).ConclusionDex has a protective effect on renal function after renal ischemia reperfusion,but this effect is inhibited in high glucose condition,which may relate to the increasing of kidney cells apoptosis.
