维持性血液透析患者的睡眠质量及相关因素

摘　　要：目的调查维持性血液透析患者的睡眠状况,了解其相关因素。方法采用匹兹堡睡眠质量指数量表(PSQI)评估血液净化中心110例维持性血液透析患者睡眠质量,并分析相关因素。收集患者性别、年龄、文化程度、原发病、透析方式等一般资料。结果维持性血液透析患者睡眠障碍发生率58%;晚间行维持性血液透析患者主观睡眠质量、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍评分均高于日间患者,差异有统计学意义(P<0.05)。每周二次血液透析、三次血液透析、二次血液透析一次血液透析滤过患者主观睡眠质量、入睡时间、睡眠效率、睡眠障碍、日间功能障碍评分呈递减关系,差异有统计学意义(P<0.05)。不同原发病患者主观睡眠质量、日间功能障碍评分比较,差异有统计学意义(P<0.05)。患者维持性血液透析时间(月龄)越长,睡眠障碍及日间功能障碍越明显。结论维持性血液透析患者睡眠质量较低,晚间血液透析患者、未进行血液透析滤过的患者、透析月龄长的患者以及原发病为糖尿病肾病患者的睡眠质量更差。Objective To investigate the sleep quality of maintenance hemodialysis(MHD)patients and its related factors.Methods110cases of MHD patients from this blood purification center were enrolled.Pittsburgh sleep quality index(PSQI)was used to assess the sleep quality of MHD patients and analyze relevant factors.The patients’general information including gender,age,degreeof education,primary disease,the way of dialysis were collected.Results The incidence of sleep disorder was58%in these MHD patients;the scores of subjective sleep quality,sleep time,sleep efficiency,sleep disorder,and daytime dysfunction in MHD patients in the evening were higher than those in the diurnal MHD patients(P<0.05).The results showed that the scores of subjective sleep quality,sleep time,sleep efficiency,sleep disorder,and daytime dysfunction were decreased in the patients who were given hemodialysis twice a week,three times a week,two hemodialysis and a hemodiafiltration a week(P<0.05).There was a significant difference for the score of the daytime dysfunction and the score of sleep disorder between different primary diseases patients(P<0.05).MHD patients with the longer duration of dialysis had the more severe sleep disorders and daytime dysfunction.Conclusions MHD patients,sleep quality is fairly lower.The sleeping quality of the patients who have hemodialysis in the evening,those who have no hemodiafiltration,those who have hemodialysis for long and those whose protopathy is diabetic nephropathy are even worse.They need to be focused.
