三通喉罩配合丙泊酚和瑞芬太尼靶控麻醉在电子支气管镜检查中的临床效果分析

摘　　要：目的:探讨三通喉罩配合丙泊酚和瑞芬太尼靶控麻醉在电子支气管镜检查中的临床效果。方法:将2014年1月~2017年2月110例电子支气管镜检查患者作为对象,依据数字表法分组,各55例。对照组通过双向靶控泵配合丙泊酚和瑞芬太尼靶控麻醉,三通喉罩组给予三通喉罩配合丙泊酚和瑞芬太尼靶控麻醉。比较两组电子支气管镜检查舒适度;电子支气管镜检查时间、通气质量评分;术前、术中、术后患者生命体征监测指标。结果:三通喉罩组电子支气管镜检查舒适度高于对照组(P<0.05)。对照组舒适有20例,比较舒适22例,不适13例,舒适度达到76.36%;三通喉罩组舒适有42例,比较舒适11例,不适2例,舒适度达到96.36%;三通喉罩组电子支气管镜检查时间(13.52±1.41)min短于对照组(24.55±2.13)min,通气质量评分(9.51±0.24)分高于对照组(7.55±0.24)分(P<0.05);术前、术后两组生命体征监测指标相近(P>0.05);术中三通喉罩组生命体征监测指标优于对照组(P<0.05)。结论:三通喉罩配合丙泊酚和瑞芬太尼靶控麻醉在电子支气管镜检查中的临床效果确切,可减轻患者疼痛等不适,提高舒适度,缩短检查时间,减少术中生命体征波动,值得推广应用。Objective This study was to investigate the clinical effect of three-way laryngeal mask with propofol and remifentanil target-controlled anesthesia in electronic bronchoscopy.Methods From January2014to February2017,110patients with electronic bronchoscopy were enrolled in this study.Fifty-five patients were divided according to the number table.In the control group,two-way target pump was used to control propofol and remifentanil target anesthesia.The three-way laryngeal mask was given with three-way laryngeal mask with propofol and remifentanil target-controlled anesthesia.The bronchoscopy time,ventilation quality score,preoperative,intraoperative and postoperative monitoring of vital signs were compared between the two groups.Results The reliability of bronchoscopy was higher than that of the control group(P<0.05).The control group was comfortable in20cases,more comfortable in22cases,13cases of uncomfortable,comfort reached76.36%;three laryngeal mask group comfortable42cases,more comfortable in11cases,2cases of discomfort,comfort reached96.36%;three throat The airway bronchoscopy time was(13.52±1.41)min shorter than the control group(24.55±2.13)min,ventilation quality score(9.51±0.24)points higher than the control group(7.55±0.24)points(P<0.05);preoperative and postoperative two groups of vital signs(P>0.05).The monitoring index of vital signs was better than that of the control group(P<0.05).Conclusion The three-way laryngeal mask with propofol and remifentanil target-controlled anesthesia in the electronic bronchoscopy in the clinical effect of accurate,can reduce the patient?s pain and discomfort,improve comfort,shorten the examination time,reduce intraoperative vital signs fluctuations,Worthy of popularization and application.
