2015年至2016年医院门急诊处方合理性分析

摘　　要：目的减少门急诊不合理处方数量,促进临床合理用药。方法对医院2015年至2016年门急诊处方进行回顾性分析。结果共抽查处方6 240张,发现不合理处方128张,其中不规范处方79张(最常见的为诊断书写不全),用药不适宜处方47张(最常见的为用法用量不适宜),超常处方2张,处方合格率为97.95%。结论该院门急诊处方基本合理,但仍需加强电子处方的监管,强化医师和药师的岗位专业培训,进一步提高临床合理用药水平。Objective To reduce the number of irrational prescriptions of Department of Outpatient and Emergency and promote rational drug use.Methods A retrospective analysis of prescription review date from2015to2016was conducted.Results Totally6240prescriptions were checked,and128irrational prescriptions were found,with79nonstandard prescriptions(the diagnosis of incomplete writing was the most common),47incompatible prescriptions(the unsuitable usage and dosage was the most common)and2extraordinary prescriptions,and the qualified rate of the prescription was97.95%.Conclusion The prescriptions are basically rational in our hospital,but still need to strengthen the management of electronic prescription,strengthen the professional training of doctors and pharmacists,and further improve the level of rational drug use.
