老年人误吸风险评估量表的临床应用

摘　　要：目的评判老年人误吸风险评估量表在预估老年患者发生误吸中的作用。方法便利选取2014年6月2016年6月在解放军第117医院住院的患者216例(平均年龄82.4±9.5岁)作为研究对象,按照随机数字法分成两组,每组108例患者,常规护理组采取标准吞咽功能评定量表(standardized swallowing assessment,SSA)、观察组应用老年人误吸风险评估量表评估误吸风险发生率,统计两组患者吸入性肺炎发生率。结果对照组吸入性肺炎发生率为49.1%(53例),观察组总发生率为22.2%(24例),其中低风险组的16例患者发生吸入性肺炎1例(6.2%),中风险组的24例患者发生吸入性肺炎4例(16.6%),高风险组28例患者发生吸入性肺炎19例患者(67.8%),组间具有统计学差异(P<0.05)。结论老年人风险误吸风险量表对老年患者误吸风险评估、分层管理有意义,可作为一种预防老年人误吸风险的预警工具。Objective To evaluate the effect of standard swallowing assessment(SSA)scale inpredicting the occurrence of aspiration in elderly patients.Methods By convenience sampling,216inpatients(average age82.4±9.5years)from2014to2016were selected and according to the random number method,patients were divided into two groups,with108cases in each group.The SSA rating scale was used in the conventional nursing group.In the observation group,the risk of aspiration was assessed according to the risk assessment scale for aged people,the incidence rates of aspiration pneumonia were compared.Results The incidence of aspiration pneumonia in the control group was49.1%(53cases).The total incidence rate in observation group was22.2%(24cases),1case developed aspiration pneumonia(6.2%)in the low risk group(16patients),4cases developed aspiration pneumonia(16.6%)in the middle risk group(24patients),19cases developed aspiration pneumonia(67.8%)in the high risk group(28patients).There were significant differences among the groups(P<0.05).Conclusion The SSA scale is more useful for risk assessment and stratification management in elderly patients and it can be used as an early warning tool to prevent the risk of aspiration.
