红花黄色素注射液联合山莨菪碱对老年ST段抬高性心肌梗死并发急性肾损伤患者血小板参数、超敏C反应蛋白及D-二聚体的影响

摘　　要：目的观察红花黄色素注射液联合山莨菪碱对老年ST段抬高性心肌梗死(STEMI)并发急性肾损伤(AKI)患者血小板参数、超敏C反应蛋白(hs-CRP)及D-二聚体的影响。方法将100例老年STEMI并发AKI患者随机分为观察组50例及对照组50例,对照组在常规治疗基础上给予山莨菪碱治疗,观察组在对照组基础上给予红花黄色素注射液治疗,2组疗程均为14 d。观察2组治疗前后血小板参数、hs-CRP、D-二聚体以及肾功能的变化情况。结果 2组治疗7 d、14 d后血小板聚集率、血小板活化因子、血小板平均体积(MPV)、血小板α颗粒膜蛋白140(GMP-140)水平及血清hs-CRP、D-二聚体水平均明显降低(P均<0.05),且观察组各指标水平均明显低于对照组(P均<0.05);2组治疗7 d、14 d后血肌酐(Cr)、尿素氮(BUN)水平与治疗前比较差异均无统计学意义(P均>0.05),但尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、尿N-乙酰-β-D氨基酸葡萄糖苷酶(NAG)浓度均明显降低(P均<0.05),且观察组明显低于对照组(P均<0.05)。结论红花黄色素注射液联合山莨菪碱能够显著抑制老年STEMI并发AKI患者血小板活化剂聚集功能,降低高凝状态及炎性反应并改善肾功能,具有一定的肾脏保护效应。It is to observe the effect of safflower yellow injection combined with Anisodamine on platelet parameters,high sensitive C reactive protein(hs CRP)and D two dimer in elderly patients with ST segment elevation myocardial infarction(STEMI)complicated with acute kidney injury(AKI).Methods100elderly patients with STEMI complicated with AKI were randomly divided into observation group(50cases)and control group(50cases),on the basis of conventional treatment,the control group was treated with Anisodamine,the observation group was treated with safflower yellow injection on the basis of the control group,14days for one course of treatment.The levels of platelet parameters,hs CRP,D two aggregates and renal function were observed before and after treatment in the2groups.Results Platelet aggregation rate,platelet activating factor,mean platelet volume(MPV),platelet alpha granule membrane protein140(GMP140)level and serum hs CRP and D two dimer levels were significantly lower in the2groups after treatment of7days and14days(all P<005),and the observation group was significantly lower than the control group(all P<0.05);there was no significant difference in serum creatinine(Cr)and urea nitrogen(BUN)levels between the2groups before and after treatment of7days and14days(all P>0.05),but urinary neutrophil gelatinase associated carrier protein(NGAL),urinary N acetyl beta D amino acid glucosidase(NAG)concentrations were significantly decreased(all P<0.05),and the observation group was significantly lower than that of the control group(P<0.05).Conclusion Safflower yellow injection combined with Anisodamine can significantly inhibit platelet aggregation function in elderly patients with STEMI complicated with AKI,reduce hypercoagulable state and inflammatory reaction and improve renal function,it has a certain protective effect on kidney.
