百令胶囊辅助恩替卡韦治疗干扰素无应答乙肝疗效观察

摘　　要：目的观察百令胶囊辅助恩替卡韦治疗干扰素无应答慢性乙型肝炎(乙肝)的临床疗效。方法将76例干扰素无应答乙肝患者随机均分为2组,恩替卡韦组给予恩替卡韦口服,联合组给予恩替卡韦联合百令胶囊口服,2组均治疗2年。分别于服药后6个月、1年和2年检测2组患者HBV DNA定量、血清HBsA g滴度、血清转化生长因子-β1(TGF-β1)、肝功能指标、氧化应激指标和免疫学相关指标,统计2组治疗2年后的临床疗效。结果治疗2年后,联合组HBV-DNA定量、血清HBsA g滴度和血清TGF-β1水平均明显低于恩替卡韦组(P均<0.05);治疗6个月、1年和2年,联合组谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酸磷酸激酶(CPK)、丙二醛(MDA)水平均明显低于恩替卡韦组(P均<0.05),超氧化物歧化酶(SOD)和还原型谷胱甘肽(GSH)水平均明显高于恩替卡韦组(P均<0.05);治疗1年和2年,联合组T淋巴细胞、单核细胞数量均明显低于恩替卡韦组(P均<0.05),血清免疫球蛋白水平均明显高于恩替卡韦组(P均<0.05);治疗2年后,联合组总有效率明显高于恩替卡韦组(P<0.05)。结论百令胶囊辅助恩替卡韦治疗乙肝可增强恩替卡韦抗病毒作用,可通过抑制HBV繁殖时的氧化应激反应、增强机体体液免疫来达到护肝降酶抗HBV作用。Objective It is to observe the clinical curative effect of Bailing Capsule assisted Entecavir in treatment of interferon unresponsive chronic hepatitis B.Methods76patients with interferon unresponsive chronic hepatitis B were randomly divided into2groups,Group A was treated with Entecavir,Group B was treated with Entecavir and Bailing Capsule,all patients were treated for2years.Serum HBV DNA quantification,serum HBsAg titer,serum transforming growth factor beta1(TGFβ1),liver function index,oxidative stress index and immunological related indexes were detected in the2groups after6months and1,2years of treatment,the clinical curative effect was counted in the2groups after2years of treatment.Results After2years of treatment,the HBV DNA quantification,serum HBsAg titers and serum TGFβ1levels in the Group B were significantly lower than those in the Group A(all P<0.05);by treated for6months,1years and2years,the alanine aminotransferase(ALT),aspartate aminotransferase(AST),and creatine kinase(CPK),malondialdehyde(MDA)levels in the Group B were significantly lower,but the levels of superoxide dismutase(SOD)and reduced glutathione(GSH)were significantly higher than those of the Group A(all P<0.05);after1and2years of treatment,the number of T lymphocytes and monocytes in the Group B were significantly lower,but serum immunoglobulin levels were significantly higher than those in the Group A(all P<0.05);after2years of treatment,the total effective rate of the Group B was significantly higher than that of Group A(P<0.05).Conclusion Bailing Capsule can enhance the anti viral effect of Entecavir in the treatment of hepatitis b,it can achieve anti liver function and anti HBV effect by inhibiting oxidative stress during HBV reproduction and enhancing body fluid immunity.
