补肾生津中药联合环磷酰胺治疗维持性血液透析继发口腔干燥症疗效及对干渴程度、透析脱水量、水通道蛋白-5的影响

摘　　要：目的观察补肾生津中药联合环磷酰胺治疗维持性血液透析继发口腔干燥症疗效及对干渴程度、透析脱水量、水通道蛋白-5的影响。方法将100例维持性血液透析继发口腔干燥症患者随机分为2组,对照组50例给予环磷酰胺治疗,观察组50例在此基础上加用补肾生津中药治疗,观察2组治疗前后中医证候积分、XQ评分、透析脱水量及水通道蛋白-5水平变化情况,统计2组近期疗效和不良反应发生情况。结果 2组治疗后口干咽燥、目赤心烦、干咳、两腮红肿、舌淡红及脉浮数积分和XQ评分、透析脱水量及水通道蛋白-5水平均显著降低(P均<0.05),且观察组治疗后以上指标均显著低于对照组(P均<0.05);观察组治疗总有效率显著高于对照组(P<0.05),但2组不良反应发生率比较差异无统计学意义(P>0.05)。结论补肾生津中药联合环磷酰胺治疗维持性血液透析继发口腔干燥症可有效缓解临床症状,减少透析脱水量,下调水通道蛋白-5水平,且未增加不良反应发生风险。Objective It is to observe the effect of Chinese medicine for tonifying kidney and promoting fluid combined with cyclophosphamide on xerostomia in maintenance hemodialysis patients,and the influence on thirst degree,dialysis water loss and aquaporin5level.Methods One hundred patients with xerostomia secondary to maintenance hemodialysis were randomly divided into2groups,50cases in the control group were treated with cyclophosphamide,and the other50cases in observation group were treated with Chinese medicine for tonifying kidney and promoting fluid.The TCM syndrome score,XQ score,dialysis water loss and aquaporin5level were observed before and after treatment in the2groups,and the short-term curative effect and adverse reactions of the two groups were analyzed.Results The integral of dry mouth and throat,red eyes and upset,dry cough,two cheek swelling,pale tongue,and pulse floating number,XQ score,dialysis water loss,and aquaporin5levels were significantly decreased in the2groups after the treatment(all P<0.05),After treatment,the above indexes in the observation group were significantly lower than those in the control group(P<0.05);The total effective rate of the observation group was significantly higher than that of the control group(P<0.05),However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:tonifying kidney and activating saliva combined with cyclophosphamide in the treatment of xerostomia secondary to maintenance hemodialysis can effectively relieve the clinical symptoms,Reducing dialysis water loss,Down regulation of aquaporin5level,There was no increase in the risk of adverse events.
