三例职业性汞中毒诊断与鉴定分析

摘　　要：三例体温表生产工人在职业健康检查中发现尿汞分别是：(627、566、494)ug/g肌酐；三例患者均有头晕、神衰、牙龈渗血、手抖，检查有伸舌震颤、上肢震颤、指鼻不准、跟膝胫欠佳、直线行走不稳等共济失调症状；其中二例患者有眼睑、伸舌、手指震颤；指鼻不准，上肢震颤，跟膝胫欠佳，直线行走不稳，其中两例，头颅MRI和肌电图显示异常。我们讨论了职业病诊断和鉴定的相关问题及汞中毒可能产生的心理创伤，建议对现有的职业病诊断鉴定方式作改进。Three employees working in a thermometer factory were examined in an occupational health examination.The mercury levels in the urine of them were627,566and494ug/g creatinine,respectively.All the three patients showed dizziness,neurasthenia,bleeding gums and tremor.Other symptoms including tongue tremor,upper limb tremor,poor heelknee-shin,and straight unsteady gait were also found.The MRI and EMG reports showed abnormalities in two of them.The issues relevant to the diagnosis and authentication of mercury poisoning,and the psychological trauma arising from the occupational disease were discussed.The advice of modifying the existing occupational disease diagnosis appraisal approach was suggested.
