摘  要:目的:探讨瞬态耳声发射(TEOAE)联合自动听性脑干反应(AABR)早期筛查新生儿高胆红素血症听力损伤的可行性及临床意义。方法:随机抽取出生3~5 d的200例高胆红素血症新生儿作为研究对象,按照筛查方法分成观察组和对照组各100例,其中对照组沿用常规的TEOAE筛选方法,观察组采用AABR+TEOAE的筛查方案,评价两组筛查通过率。结果:观察组检出率要明显高于对照组(P<0.05),其中最终确诊的5例高胆红素血症听力损伤患儿中,对照组筛查未通过1例,而观察组一次检出率80.0%。结论:AABR是早期筛查新生儿听力比较好的一种方法,联合TEOAE能进一步提高灵敏度和筛查率,及早发现新生儿高胆红素血症耳蜗后病变,为提前干预奠定基础。Objective:To investigate the feasibility and clinical significance of transient otoacoustic emissions(TEOAE)combined with automatic auditory brainstem response(AABR)in early screening of hearing impairmen in neonatal hyperbilirubinemia.Methods:200cases of neonatal hyperbilirubinemia from3to5D Who were randomly selected as the research object,were divided into the observation group and the control group with100cases in each group according to the screening method.The patients in the control group were used routine TEOAE screening method,while those in the observation group were used AABR+TEOAE screening program.Two groups of screening pass rate were evaluated.Results:The detection rate of the observation group was higher than that of control group(P<0.05),in which5cases of hyperbilirubinemia in children with hearing impairment were eventually diagnosed.Otherwise,the control group did not pass the screening of1case,and in the observation group,a detection rate of80%was fulfilled.Conclusions:AABR is a better method for early screening of newborn hearing.The transient otoacoustic emissions(TEOAE)combined with automatic auditory brainstem response(AABR)can further improve the sensitivity and the screening rate,which makes it possible to early detect neonatal hyperbilirubinemia posterior cochlear lesions with the foundation for early intervention.
