胎儿头颈部血管瘤产前超声表现及临床结局研究

摘　　要：[目的]深入探讨产前胎儿头颈部血管瘤的超声表现及其临床结局。[方法]回顾性分析2014年1月至2016年12月期间我院收治的26例疑似胎儿头颈部血管瘤患者的基本临床资料及超声声像特征。[结果]除了1例诊断为淋巴管囊肿外,其余25例均确诊为头颈部血管瘤。超声表现为囊实性混合回声为16例、实性回声肿块9例。25例胎儿家属均选择继续妊娠,在胎儿出生后未出现死亡或心力衰竭等情况,预后良好。[结论]胎儿头颈部血管瘤产前超声主要特征为囊实性混合回声型或实性回声型,该病通常可自行消退,对胎儿一般不产生不良影响。但对于诊断过程中出现血管瘤较大者,应每间隔2周对胎儿进行一次心功能检查,预防引发心功能异常等并发症。〔Objective〕Discuss deeply the ultrasonographic features and clinical outcomes of fetal head and neck hemangioma.〔Methods〕Methods the clinical data and ultrasonographic features of26cases of suspected fetal head and neck hemangioma in our hospital from January2014to December2016were analyzed retrospectively.〔Results〕Except for1cases were diagnosed as lymphatic cysts,the other25cases were diagnosed as head and neck hemangioma.The sonographic findings were solid cystic mixed echo in16cases and solid echo mass in9cases.25case of families of the fetus chose to continue the pregnancy,there is no death or heart failure and the prognosis is good after the fetus is born.〔Conclusion〕The main features of prenatal ultrasound in the head and neck hemangioma of the fetus are cystic and solid mixed echo or solid echo.The disease usually resolves by itself and has no adverse effects on the fetus.However,in the course of the diagnosis of hemangioma,should be2weeks intervals for fetal heart function test,prevention of cardiac dysfunction and other complications.
