婴幼儿先天性鼻泪管阻塞的保守治疗研究

摘　　要：目的对比研究泪囊按摩与泪道冲洗治疗对6月龄至15月龄婴幼儿先天性鼻泪管阻塞(congenitalnasolacrimalductobstruction,CNLDO)的疗效。方法2013年1月至2015年12月在本院被眼科医生诊断为先天性鼻泪管阻塞6月龄至15月龄的19例(22眼)婴幼儿,患儿根据不同治疗方法分为两组,泪囊按摩组11例(13眼),和加压泪道冲洗组8例(9眼),从纳入研究随访至治愈或者年龄达到15月龄,观察对比两组治愈率、治愈时间有无差别。结果泪囊按摩组11例(13眼),愈合11只眼,总治愈率约为85%,其中5眼在7~9月愈合(约45%),4眼在10~12月愈合(约36%),2眼在13月至15月愈合(约18%);加压泪道冲洗组8例(9眼),8眼愈合,总治愈率为89%,6眼在7~9月愈合(75%),1眼在10~12月愈合(约13%),1眼在13~1s5月愈合(约13%),两组治愈率差别无统计学意义(χ2=0.082,P=0.774),两组间不同月龄愈合例数差异也无明显统计学意义(χ2=1.795,P=0.408),平均治愈时间泪囊按摩组为9.8±2.1月,泪道冲洗组为9.1±1.7月,差别也无统计学意义(F=1.065,P=0.317)。结论合理的保守治疗对于先天性鼻泪管阻塞有着积极临床意义,可以推迟非保守治疗时间。ObjectiveTo compare the efficacy of lacrimal sac massage and pressor clysis of congenital nasolacrimal duct obstruction(CNLDO)in infants.Methods19infants(22eyes)aged0.5to15months who had CNLDO,according to the lacrimal sac massage or pressor clysis in common lacrimal passage treatment from January2013to December2015,the infants were diviede into two group,lacrimal sac massage group11cases(13eyes),pressor clysis group8cases(9eyes),all the case were followed up till resolution of CNLDO or aged15months,efficacy of different treatmen and the time of resolution of CNLDO in two groups were compared respectively.ResultsThe success rate of lacrimal sac massage group was85%,while the pressor clysis group was89%,in lacrimal sac massage group5eyes resoluted in7~9months(about45%),4eyes in10~12months(about36%),2eyes in13~15months(18%),while in pressor clysis group6eyes resoluted in7~9months(75%),1eye in10~12months(about13%),1eye in13~15months(about13%).There was no significant difference in cure rate,cure cases in different age(χ2=0.082,1.795;P=0.774,0.408respectively).And time of resolution lacrimal sac massage group was9.8±2.1months,pressor clysis group9.1±1.7months,there also was no significant difference between two groups(F=1.065,P=0.317).ConclusionThere is meanful clinical signifcance of resonable conservative treatment in infants CNLDO,the time of unconservative treatment should be put off.
