临床药师干预钻颅血肿外引流术、骨折内固定物取出术围手术期预防使用抗菌药物的对比分析

摘　　要：目的:评价我院临床药师对钻颅血肿外引流术、骨折内固定物取出术两种清洁手术围手术期预防使用抗菌药物进行干预后的效果。方法:回顾性调查2012年1月至2013年12月出院的钻颅血肿外引流术、骨折内固定物取出术手术病历共366例(干预前)和2015年1月至2016年12月出院的钻颅血肿外引流术、骨折内固定物取出术手术病历共350例(干预后),针对围手术期抗菌药物预防应用的合理性进行评价。结果:通过临床药师参与钻颅血肿外引流术、骨折内固定物取出术围手术期预防性抗菌药物的使用,干预前、干预后抗菌药物预防使用比例分别为89.3%和32.3%(P<0.05),抗菌药物预防用药不超过24h的比例分别为14.4%、86.8%(P<0.05),预防性使用抗菌药物的合理率分别为10.7%、67.7%(P<0.05)。结论:临床药师对围手术期预防性使用抗菌药物进行干预,能提高抗菌药物使用的合理率,降低药品费用,对促进临床安全、有效、经济的使用抗菌药物具有良好的效果。Objective:To investigate the effect of clinical pharmacists intervene in prophylactic use of antimicrobial agents in2kinds of typeⅠincision operation during perioperative period.The2kinds of typeⅠincision operation include sphenotresia drainage and internal flxation removal surgery.Methods:The clinical data of patients who were in hospital with sphenotresia drainage and internal flxation removal surgery were retrospectively analyzed.366patients were non-intervention group from Jaunary2012to December2013,and there were350patients in the intervention group from Jaunary2015to December2016.The medication rationality in prophylactic use of antimicrobial agents during perioperative period were statistically analyzed.Results:Through the clinical pharmacists intervene the sphenotresia drainage and internal flxation removal surgery in prophylactic use of antimicrobial agents during perioperative period,the usage rates of antimicrobial agents in the non-intervention group and the intervention group were respectively89.3%and32.3%(P<0.05);the percentages of drug duration less than24h were14.4%and86.8%(P<0.05);the rates of rationality were10.7%、67.7%(P<0.05).Conclusion:The clinical pharmacists intervene in prophylactic use of antimicrobial agents during perioperative period can significantly improve the rational use of antimicrobial drugs,and improve clinical safety,effective and economical use of antimicrobial drugs.
