摘　　要：目的构建高警示药品医嘱识别能力调查问卷,为了解临床护理人员的高警示药品医嘱识别能力现状提供客观、科学的评价工具。方法在专家访谈和文献回顾的基础上,结合临床经验,形成备选条目,采用德尔菲法对24名专家进行三轮函询,拟定调查问卷,并分析问卷的信效度。结果专家的积极性较高,第1轮、第2轮、第3轮的专家积极系数分别为100%、95.8%、100%,专家权威系数分别为0.88、0.87、0.90;三轮咨询后,专家意见的协调系数为0.34(P<0.0001);问卷的Cronbach’sα系数为0.739,CVI为0.964;最终形成了临床护理人员的高警示药品医嘱识别能力调查问卷。结论高警示药品医嘱识别能力问卷信效度较好,可作为临床护理人员高警示药品医嘱识别能力的评价工具。Objective To develop the doctor’s advice recognition questionnaire of high alert medications for clinical nurses and to provide a scientific and objective measurement tool for investigating the clinical medical staff’s doctor’s advice recognition of high alert medications.Methods Based on experts interview and a literature review,combined with clinical experiences,formed the alternative entry.Delphi technique had been used to investigate24experts in this field for three rounds.Then the questionnaire was formulated.Then we analyzed the reliability and the validity of the questionnaire.Results The experts’positivity was high and the response rates of the three round consultation were100%、95.8%and100%,authority coefficients were0.88、0.87and0.90,Kendalls’W was0.34(P<0.0001)after the three-round consultation.The Cronbach’sαof the questionnaire was0.739and CVI was0.964.Developed the doctor’s advice recognition questionnaire of high alert medications for clinical nurses.Conclusion The questionnaire has good reliability and validity and it could be used to investigate the doctor’s advice recognition questionnaire of high alert medications for clinical nurses.
