摘  要:目的:探讨全模型迭代重组(IMR)结合体质量指数(BMI)指导下在肾脏低剂量CT扫描中的图像质量及可行性。方法:36例21≤BMI≤25 kg/m2肾脏平扫患者采用80 k V,自适应管电流技术,IMR重建,作为实验A组(80 k V/IMR重建);35例21≤BMI≤25 kg/m2肾脏平扫患者采用100 k V,自适应管电流技术,IDose重建,作为常规B组(100 k V/IDose重建)。测量2组图像的肾脏的噪声,信噪比(SNR),对比噪声比(CNR)并记录CT剂量指数(CTDIvol)计算有效辐射剂量(ED),两名高年资诊断医师采用双盲法对图像质量进行客观测量及主观评价。采用独立样本t检验对结果进行比较,两名医师的一致性评估采用Kappa检验。结果:A组和B组的噪声分别为(4.92±0.73)HU和(10.30±1.37)HU,差异具有统计学意义(t=-7.96,P<0.05)。A组和B组的SNR分别为9.29±2.84和3.97±1.78,差异具有统计学意义(t=3.547,P<0.05)。A组和B组的CNR分别为43.38±11.52和16.36±7.94,差异具有统计学意义(t=4.352,P<0.05)。A组和B组的有效辐射剂量分别为(0.673±0.066)m Sv和(1.314±0.298)m Sv,差异有统计学意义(t=-4.698,P<0.05)。结论:Philips Brilliance i CT 256层CT低管电压(80 k V)结合IMR重组技术在肾脏CT平扫客观和主观图像质量综合评价中优于常规i Dose图像,并且明显降低患者所受辐射剂量,具有很好的应用价值和前景。Objective The discussion iteration reorganizes under the IMR union body mass index(BMI)instruction image quality and feasibility in kidney low dose CT scanning.Method36cases of21≤BMI≤25kg/m2kidney draws sweep patient to use80kV,auto-adapted tube electric current technology,IMR reconstruction,as testing A groups(80kV/IMR reconstruction)[Hereafter refers to as A groups];35cases of21≤BMI≤25kg/m2kidney draws sweep patient to use100kV,auto-adapted tube electric current technology,IDose reconstruction,as conventional B groups(100kV/IDose reconstruction)[Hereafter refers to as B groups].Surveys the noise of2pictures alike kidney,signal-to-noise ratio(SNR),contrast noise ratio(CNR)and records CT dosage index(CTDIvol)to calculate effective dose(ED),two high periods of service diagnose the doctor to use the blind law to carry on the objective measurement and subjective evaluation to the image quality.Compares result with the independent sample T-test,the uniform appraisals of two doctors use the Kappa examination.Results The noises of A group and B group respectively was(4.92±0.73)HU and(10.30±1.37)HU,the difference was statistically significant(t=-7.96,P<0.05).SNR of A group and B group respectively was9.29±2.84and3.97±1.78,the difference was statistically significant(t=3.547,P<0.05).CNR of A group and B group respectively was43.38±11.52and16.36±7.94,the difference was statistically significant(t=4.352,P<0.05).The effective doses of A group and B group respectively was(0.673±0.066)mSv and(1.314±0.298)mSv,the difference was statistically significant(t=-4.698,P<0.05).Conclusion Philips Brilliance iCT256CT(80kV)in kidney Ping Sao,compare with the conventional dosage IDose reconstruction,IMR reconstruction technology and BMI instruction the integration under of low dose technology,under guaranteeing the premise of image quality,effectively reduced the radiation dose,it has good application value and prospect.
