关节镜辅助内侧副韧带浅层松解联合玻璃酸钠注射液对内侧半月板损伤修复效果分析

摘　　要：目的分析关节镜辅助内侧副韧带浅层松解联合玻璃酸钠关节腔内注射对内侧半月板损伤的修复效果。方法采用随机数表法将2015年2月至2016年2月于本院治疗的58例内侧半月板损伤患者分为试验组和对照组,每组各29例,两组患者均行关节镜辅助内侧副韧带浅层松解术,试验组患者术后定期行关节腔内玻璃酸钠注射治疗,术后1个月、6个月及12个月来院复查,比较两组患者膝关节症状及体征阳性率、Lysholm评分、国际膝关节评分委员会(International Knee Documental Commit-tee,IKDC)膝关节主观功能评分及疗效,并通过MRI判断内侧半月板愈合情况。结果术后1个月,两组患者膝关节Lysholm评分和IKDC评分均无显著差异(P>0.05);术后6个月和12个月,试验组患者膝关节Lysholm评分和IKDC评分均显著高于对照组(P<0.05)。术后1个月、6个月及12个月,试验组患者膝关节症状体征阳性率均显著低于对照组(P<0.05),且试验组患者内侧半月板愈合率和治疗显效率均显著高于对照组(P<0.05),两组患者治疗有效率无显著差异(P>0.05)。结论关节镜辅助内侧副韧带浅层松解联合玻璃酸钠注射治疗,可快速促进内侧半月板修复,有效改善膝关节不良症状及体征,提高治愈效果和膝关节功能。Objective To explore the effect of pie-crusting technique of superficial medial collateral ligament under arthroscopic combined with sodium hyaluronate in repairing medial meniscus injury.Method From February2015to February2016,58patients with medial meniscus injury were divided into experimental group and control group according to the random number method,29cases in each group.Two groups of patients adopted arthroscopic assisted superficial medial collateral ligament release,while experimental group patients adopted sodium hyaluronate after operation.All patients were followed up for1month,6months and12months,and compared two groups with the positive rates of knee joint symptoms and signs,Lysholm score,International Knee Documental Commit-tee(IKDC)score and treatment effects,then determined the medial meniscus healing situation by MRI.Result There was no significant difference in Lysholm scores and IKDC scores between two groups at1month after operation(P>0.05).But at6months and12months after operation,the Lysholm scores and IKDC scores of experimental group were significantly higher than control group(P<0.05).The positive rates of knee symptoms and signs in experimental group were lower than control group at1month,6months and12months after operation(P<0.05).The meniscus healing rate and treatment efficiency rate in experimental group were significantly higher than control group(P<0.05),but there was no significant difference between two groups in treatment effect rates(P>0.05).Conclusion Arthroscopic assisted superficial medial collateral ligament release combined with sodium hyaluronate can promote medial meniscus repairing effectively,improve the symptoms and signs of knee malformations,improve healing effect and knee function.
