联合应用降钙素原、C-反应蛋白和白细胞计数在妊娠合并急性阑尾炎治疗中的应用价值

摘　　要：目的探讨血清降钙素原(PCT)、C-反应蛋白(CRP)和白细胞计数(WBC)联合应用对妊娠合并急性阑尾炎中的临床价值。方法回顾2009年3月至2015年2月收治的81例妊娠合并急性阑尾炎患者资料,收集PCT、CRP、WBC三项指标检测结果和治疗用药、母婴并发症情况,并进行分析。结果单纯性阑尾炎(SA)24例,化脓性阑尾炎(PA)37例,坏疽穿孔性阑尾炎(GA)11例,阑尾周围脓肿(AA)9例。这4种病理类型的PCT、CRP、WBC检测结果比较,差异具有统计学意义(P<0.05)。81例患者中切口感染率为3.7%(3/81),无腹腔残余脓肿、下肢静脉血栓等其他手术并发症,胎儿丢失率为2.7%(2/74)。结论联合应用PCT、CRP、WBC三项指标对妊娠合并急性阑尾炎的病理分型、治疗策略以及减少母婴并发症具有重要指导意义。Objective To explore the application value of PCT combined with CRP and WBC in the treatment of pregnancy complicated with actue appendicitis.Methods The clinical data of eighty-one cases of pregnancy complicated with acute appendicitis from March2009to February2015were collected.The data including three test indexes of PCT,CRP,WBC and treatment,maternal and infant complications were analyzed.Results There were24cases of simple appendicitis(SA),37cases of suppurative appendicitis(PA),11cases of gangrene perforated appendicitis(GA),9cases of abscess around the appendix(AA)among them.The differences in the detection results of PCT,CRP and WBC in the4kinds of pathological types were statistically significant(P<0.05).The incisional infection rate was3.7%(3/81)in81patients.There were no abdominal residual abscess,lower extremity venous thrombosis and other complications.The fetal loss rate was2.7%(2/74).Conclusion The PCT combined with CRP and WBC in the treatment of pregnancy with acute appendiciti has important guiding significance for evaluating the pathological classification,the treatment strategy and reducing maternal and infant complicationst.
