静脉铁剂治疗慢性心力衰竭并发铁缺乏患者的疗效观察

摘　　要：目的通过观察静脉使用铁剂在慢性心力衰竭(chronic heart failure,CHF)并发铁缺乏(iron deficiency,ID)患者治疗中的疗效,进一步明确静脉使用铁剂对CHF并发ID患者的生活质量、心功能、运动耐量的改善情况。方法选取CHF并发ID的患者为研究对象。共纳入101例,随机分为静脉铁剂治疗组51例,对照50例。两组均给予常规药物个体化治疗,静脉铁剂治疗组在常规药物个体化治疗的基础上给予静脉注射蔗糖铁注射液10 m L,每周1次。观察两组血红蛋白(hemoglobin,Hb)浓度、血清铁蛋白(serum ferritin,SF)浓度、心功能、活动耐力及生活质量。结果观察组治疗后的Hb浓度、SF浓度、6 min步行距离(6-minute walkdistance,6MWD)、心功能、明尼苏达心力衰竭生活质量调查表(Li HFe)得分比治疗前提高;对照组治疗后Hb浓度、SF浓度比治疗前降低,差异均有统计学意义(P<0.05)。结论静脉补充铁剂可明显改善CHF并发ID患者的运动耐量、生活质量、心功能。Objectives To observe the improvement of quality of life,cardiac function,exercise tolerance in patients with chronic heart failure(CHF)and iron deficiency(ID)treated by intravenous iron therapy.Methods Totally101patients with CHF and ID were enrolled in this study.The patients were randomly divided into intravenous iron treatment group(n=51)and control group(n=50).Patients in the two groups were treated with conventional drugs individualized treatment,intravenous iron treatment group received intravenous iron sucrose injection10mL once a week based on routine drugs.Concentrations of hemoglobin(Hb),serum ferritin(SF),heart function,quality of life and endurance activities were observed and compared between the two groups.Results Concentrations of Hb,SF and6-minute walk distance(6MWD),heart function,score of quality of life questionnaire for heart failure in Minnesota(LiHFe)in treatment group after treatment were significantly higher than those before treatment;concentrations of Hb and SF in control group after treatment were significantly lower than those before treatment(P<0.05).Conclusions Intravenous iron supplements can significantly improve exercise tolerance,quality of life,heart function in patients with CHF and ID.
