摘 要:目的:检测呼出气一氧化氮(FENO)和呼出气冷凝液(EBC)中白介素-17(IL-17)在慢性阻塞性肺疾病(简称慢阻肺)发病过程及严重程度判断中的意义。方法:收集81例慢阻肺急性加重期患者、经14 d左右正规抗感染、对症处理病情明显缓解后视为稳定期,78例健康者为对照组,并行肺功能检查,FENO使用一氧化氮测定仪测定,EBC中IL-17利用酶标法检测。结果:(1)慢阻肺急性加重期组中FENO、IL-17水平都高出稳定期,差异有统计学意义(P<0.05);(2)慢阻肺稳定期组中FENO、IL-17水平都高出对照组,差异有统计学意义(P<0.05);(3)慢阻肺急性加重组中FENO、IL-17水平都高出对照组,差异有统计学意义(P<0.05);(4)慢阻肺急性加重期组患者FENO水平和FEV1%都无相关性,差异无统计学意义(P>0.05);慢阻肺稳定期组患者FENO水平和FEV1%都无相关性,差异无统计学意义(P>0.05);FENO从急性加重期到稳定期改善程度和FEV1%从急性加重期到稳定期的改善程度有正相关关系,差异有统计学意义(P<0.05);慢阻肺急性加重期及稳定期FENO与IL-17都有显著正相关关系,差异有统计学意义(P<0.05)。结论:慢阻肺的发病机制中FENO、IL-17也许有关键影响,两者联合也许可以作为判断慢阻肺病情严重程度的重要方法。Objective Detection of breath(fraction of exhaled nitric oxide nitric oixde FENO)and breath condensate(exhaled breath condensate,EBC)of interleukin-17(IL-17)the clinical significance of chronic obstructive pulmonary disease.Analysis of hospitalized patients with AECOPD FENO value and IL-17and curative effect before and after the treatment.Method In81cases of COPD exacerbations period,81cases of COPD stabilization,78cases of healthy subjects as the research object,the parallel routine lung function,with Sweden,resistant NIOX nitrous oxide FENO meter detection.By using enzyme-linked immunosorbent(ELSIA)determination of the level of IL-17in EBC.Results①in acute aggravating period of COPD group FENO,IL-17levels were higher than in plateau(P<0.05);②the COPD group stability FENO,IL-17levels were higher than control group(P<0.05);③with COPD and restructuring of FENO,IL-17levels were higher than control group(P<0.05);④correlation study found:COPD patients with acute aggravating period group FENO level had no correlation with FEV1%(P>0.05);Patients with COPD stabilization group FENO level had no correlation with FEV1%(P>0.05);Acute exacerbation period to stabilization FENO improve value with acute aggravating period to stability improved FEV1%value also exists between positive correlation(P<0.05).FENO and IL-17were positively correlation between acute and stability group(P<0.05).Conclusion FENO,IL-17May occur in COPD,and play an important role in the process of development,may become the index reflecting the severity of COPD patients.
