乳腺钼靶X线与彩超联合检查对于乳腺导管内癌的诊断价值

摘　　要：目的分析乳腺导管内癌(DCIS)的钼靶X线与彩色多普勒超声特征,并探讨乳腺钼靶X线与彩超联合检查对于DCIS的诊断价值。方法选取2014年1月-2016年4月我院收治的经病理证实的63例DCIS患者作为研究对象,对所有患者的乳腺钼靶X线与彩色多普勒超声影像学资料进行回顾性分析。结果 63例乳腺导管内癌患者双乳共检出并经病理检查确诊有84个病灶,其中导管内癌63个,良性肿瘤21个。乳腺钼靶+彩超诊断的敏感性、阴性预测值分别为93.65%、83.33%,均显著高于乳腺钼靶、彩超单独诊断(P<0.05);乳腺钼靶+彩超诊断的特异性为95.24%,显著高于乳腺钼靶(P<0.05)。乳腺钼靶检查时回声/密度不均匀、边界模糊检出率分别为79.37%、60.00%,均显著低于彩超(P<0.05);乳腺钼靶检查时微小钙化检出率为76.09%,显著高于彩超(P<0.05)。结论乳腺钼靶X线与彩超联合检查能有效提高DCIS诊断的准确性,具有较高的应用价值。Objective To analyze the characteristics of mammography and color Doppler ultrasonography in mammary ductal carcinoma(DCIS),and the diagnostic value of breast mammography X-ray and color Doppler ultrasonography for DCIS.Methods A total of63patients with DCIS were enrolled in our hospital from January2014to April2016.The mammography mammography and color Doppler ultrasonography were retrospectively analyzed for all patients.Results63cases of breast ductal cancer patients were diagnosed with double milk and confirmed by pathology84lesions,including63cases of ductal cancer,21cases of benign tumors.The sensitivity and negative predictive value of breast mammography and color Doppler ultrasonography were93.65%and83.33%,respectively,which were significantly higher than those of mammography and color Doppler ultrasonography(P<0.05).The specificity of breast mammography+color Doppler ultrasonography was95.24%,which was significantly higher than that of mammogra phy(P<0.05).The detection rate of the boundary blurred was79.37%and60.00%,respectively,which were significantly lower than those of the color Doppler ultrasonography(P<0.05).The detection rate of microcalcification was76.09%,which was significantly higher than that of color Doppler ultrasonography(P<0.05).Conclusion Breast mammography combined with color Doppler ultrasonography can improve the accuracy of DCIS diagnosis and have high application value.obviously higher than that in control group(P<0.05).
