摘 要:目的探讨急性心肌梗死(AMI)患者病程与心率减速力的相关性。方法选取2011年1月—2015年5月在武汉市第一医院接受治疗的AMI患者91例,根据病程将其分为:A组(病程<1个月,n=28),B组(病程为1~3个月,n=31),C组(病程为4~6个月,n=16),D组(病程>6个月,n=16);另选取同期在本院接受体检的健康者92例,为对照组。比较5组心率减速力(DC)以及连续心率减速力(DRs)中的DR2、DR4、DR8。采用Spearman秩相关分析AMI患者病程与DC、DRs的相关性。结果 5组DC及DR2、DR4、DR8水平比较,差异有统计学意义(P<0.05)。DC、DR4、DR8与AMI患者病程呈正相关(r_s值分别为0.546、0.614、0.324,P<0.05),DR2与AMI患者病程无相关关系(r_s=0.088,P>0.05)。结论 AMI患者病程越短,DC值较低,迷走神经功能活性降低,猝死风险越高。Objective To investigate the regularity of changes in deceleration capacity(DC)of heart rate in patients with different duration of acute myocardial infarction(AMI).Methods The enrolled participants were all from Wuhan No.1 Hospital,including 91 cases of AMI(AMI group)receiving treatment from January 2011 to May 2015 and 92 healthy controls(control group)underwent physical examination during the same period.Based on the duration of AMI,AMI patients were divided into subgroup A(<1 month,n=28),subgroup B(1-3 months,n=31),subgroup C(4-6 months,n=16),and subgroup D(>6 months,n=16).The DC and DR2,DR4 and DR8 these three consecutive heart rate deceleration runs(DRs)were compared between the groups.Spearman correlation analysis was performed to investigate the correlation beteween different duration of AMI and DC,DR.Results The value of DC and DRs,as well as DRs of cycles differed significantly between the controls and four AMI subgroups(P<0.05).The value of DC,DR4 and DR8 increased with the prolongation of the duration of AMI(rs=0.546,0.614,0.324,P<0.05).The value of DR2 did not correlate with the duration of AMI(rs=0.088,P>0.05).Conclusion The value of DC decreased with the duration of AMI,leading to decreased activity of the vagus nerve and the higher the risk of sudden death.
