中药方剂对妊娠期糖尿病氧化应激状态、细胞粘附因子及妊娠结局的影响

摘　　要：目的:探讨中药方剂"孕糖方"对妊娠期糖尿病患者氧化应激状态以及细胞粘附因子(s ICAM-1)水平的改变,探索中药治疗妊娠期糖尿病对母婴结局的影响。方法:选取2014年1月-2016年1月确诊为妊娠期糖尿病(GDM)患者60例,其中35例以口服中药方剂加饮食控制治疗(孕糖方组),25例以饮食控制治疗(非中药组),另选正常妊娠(对照组)30例,留取孕妇静脉及胎儿脐静脉血清,测定超氧化物歧化酶(SOD)、脂质氧化物(MDA)含量;测定细胞间粘附因子(ICAM-1)含量,对三组数据作统计学比较,同时比较三组母婴不良结局的发生率。结果:妊娠期糖尿病患者存在明显的氧化应激失衡,孕糖方组孕妇血清和脐血中MDA浓度显著低于非中药组,两组均高于对照组(P<0.05),超氧化物歧化酶SOD浓度孕糖方组显著高于非中药组及对照组(P<0.05),孕糖方组s ICAM-1水平显著高于非中药组及对照组(P<0.05)。临床应用中药方剂治疗妊娠期糖尿病,母婴并发症有所减少。结论:妊娠期糖尿病存在氧化应激失衡,造成不良妊娠结局的原因与氧化应激及血管内皮损伤密切相关。中药"孕糖方"可改善妊娠期糖尿病患者的氧化应激失衡,从降低血管内皮损伤角度改善妊娠期糖尿病患者的妊娠结局。Objective:To investigate the efficacy of the Yuntang prescription on oxidative stress,sICAM-1and pregnancy outcomes of women with GDM.Methods:60cases were divided into two groups.35cases in the Yuntang prescription group were treated with the Yuntang prescription plus dietary management;25cases in the non-TCM medicine group were given dietary management only.Then30cases of normal pregnant women were served as the control group.The levels of SOD,MDA and sICAM-1were determinated by venous serum of pregnant women of fetal umbilic;pregnancy outcomes in groups were compared.Result:Oxidative stress of GDM was an obvious imbalance.MDA in the Yuntang prescription group was significantly lower than the non-TCM medicine group,and that of60GDM patients was higher than the control group(P<0.05).The sICAM-1and SOD in the Yuntang prescription group was significantly higher than the non-TCM medicine group and the control group(P<0.05).TCM medicne on GDM can reduce complication.Conclusion:GDM can lead to imbalance of oxidative stress and cause adverse pregnancy outcomes,which is closely related to oxidative stress and vascular endothelial injury.The Yuntang prescription can improve oxidative stress imbalance,reduce vascular endothelial injury and improve pregnancy outcomes.
