炙甘草汤联合黄连温胆汤加味治疗病毒性心肌炎后遗症期患者的临床疗效

摘　　要：目的观察炙甘草汤联合黄连温胆汤加味治疗病毒性心肌炎(VMC)后遗症期患者的临床疗效。方法选取西安交通大学第一附属医院2014年3月—2017年3月收治的VMC后遗症期患者82例,随机分为对照组和观察组,每组41例。对照组患者给予曲美他嗪和辅酶Q10治疗,观察组患者在对照组基础上给予炙甘草汤联合黄连温胆汤加味治疗;4周为1个疗程,两组患者均治疗2个疗程。比较两组患者治疗前后中医证候评分,治疗1个疗程及2个疗程后临床疗效,治疗前后血清乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平,并记录两组患者治疗期间不良反应发生情况。结果治疗前两组患者主症总评分、次症总评分及中医证候总评分比较,差异无统计学意义(P>0.05);治疗后观察组患者主症总评分、次症总评分及中医证候总评分低于对照组(P<0.05)。两组患者治疗1个疗程及2个疗程后临床疗效比较,差异均无统计学意义(P>0.05)。两组患者治疗前血清LDH、CK、CK-MB水平比较,差异无统计学意义(P>0.05);治疗后观察组患者血清LDH、CK、CK-MB水平低于对照组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论炙甘草汤联合黄连温胆汤加味治疗能有效改善VMC后遗症期患者中医证候,减轻心肌损伤,且安全性较高。Objective To observe the clinical effect of Zhi Gancao Decoction combined with modified Huanglianwendan Decoction in treating viral myocarditis sequela stage patients.Methods A total of82viral myocarditis sequela stage patients were selected in the First Affiliated Hospital of Xi'an Jiaotong University from March2014to March2017,and they were randomly divided into control group and observation group,each of41cases.Patients in control group received trimetazidine combined with coenzyme Q10,while patients in observation group received Zhi Gancao Decoction combined with modified Huanglian-wendan Decoction based on that of control group;both groups continuously treated for2courses(4weeks as a course).TCM syndrome score,serum levels of LDH,CK and CK-MB before and after treatment,clinical effect1course and2courses after treatment were compared between the two groups,and incidence of adverse reactions was observed during the treatment.Results No statistically significant differences of total main symptom score,total minor symptom score or total TCM syndrome score was found between the two groups before treatment(P>0.05),while total main symptom score,total minor symptom score and total TCM syndrome score in observation group were statistically significantly lower than those in control group after treatment(P<0.05).No statistically significant differences of clinical effect was found between the two groups1course or2courses after treatment(P>0.05).No statistically significant differences of serum level of LDH,CK or CK-MB was found between the two groups before treatment(P>0.05),while serum levels of LDH,CK and CK-MB in observation group were statistically significantly lower than those in control group after treatment(P<0.05).No statistically significant differences of incidence of adverse reactions was found between the two groups during the treatment(P>0.05).Conclusion Zhi Gancao Decoction combined with modified Huanglian-wendan Decoction can effectively relieve the TCM syndrome and myocardial damage in treating viral
