摘 要:目的评价高频振荡通气对新生儿持续肺动脉高压治疗的有效性及安全性。方法 2013年10月~2016年10月我院新生儿持续肺动脉高压患儿60例,按治疗方法分两组,对照组采用常频机械通气治疗,观察组采用高频震荡通气治疗。结果 (1)与对照组比较,观察组并发症发生率明显更低,平均住院时间和通气时间明显更短,差异有统计学意义(P<0.05)。(2)与对照组比较,观察组治疗后pH值明显更高,PaO_2明显更高,PaCO_2明显更低,呼吸频率和心率明显更低,差异有统计学意义(P<0.05)。与治疗前比较,治疗后观察组pH值明显升高,PaO_2明显升高,PaCO_2明显降低,呼吸频率和心率明显降低,差异有统计学意义(P<0.05)。(3)与对照组比较,观察组治疗有效率明显更高,差异有统计学意义(P<0.05)。(4)观察组治疗后LVEDD、LVESD、LVEDV、LVESV水平明显比对照组治疗后更低,比治疗前降低,差异有统计学意义(P<0.05)。(5)观察组治疗后PEFR、MVV比治疗前明显升高,Raw比治疗前明显降低,PEFR、MVV比对照组明显更高,Raw比对照组明显更低,差异有统计学意义(P<0.05)。结论高频振荡通气对新生儿持续肺动脉高压治疗的有效性显著,安全性良好,值得临床广泛应用以及推广。Objective To evaluate of the efficacy and safety of high-frequency oscillatory ventilation in the treatment of neonatal persistent pulmonary hypertension.Methods From October2013to October2016,60neonates with persistent pulmonary hypertension in the people's Hospital of Boluo County,Huizhou City,were divided into three groups:the control group was treated with mechanical ventilation of normal frequency,and the observation group was treated with high frequency concussion ventilation.Results(1)Compared with the control group,the incidence of complications in the observation group was significantly lower,the average hospitalization time and ventilation time were significantly shorter than those in the control group(P<0.05).(2)the pH value of the observation group was significantly higher,the PaO2was significantly higher,the PaCO2was significantly lower,and the respiratory frequency and heart rate were significantly lower,P<0.05.Compared with those before treatment,the PH value of the observation group increased significantly after treatment.PaO2increased significantly and Paco2decreased significantly,and the respiratory frequency and heart rate decreased significantly(P<0.05).(3)Compared with the control group,the effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).(4)After treatment,the LVEDDV,LVESD,LVEDVV and LVESV levels in the observation group were significantly lower than those in the control group(P<0.05).(5)After treatment,the PEFR,MVVV of the observation group were significantly higher than that of the control group(P<0.05),and that of Raw was significantly lower than that of the control group(P<0.05),PEFRV and Raw were significantly higher and lower than those in control group(P<0.05),Raw was significantly lower than that in the control group(P<0.05).Conclusion High frequency oscillatory ventilation is effective and safe in the treatment of neonates with persistent pulmonary hypertension.It is worthy of clinical application and popularization
