超声介入下聚桂醇注射联合清宫术与MTX肌注联合清宫术治疗剖宫产瘢痕妊娠的疗效对比观察

摘　　要：目的探讨B型超声引导下聚桂醇剖宫产瘢痕妊娠病灶局部注射联合清宫术治疗剖宫产瘢痕妊娠的临床疗效。方法选择我院2016年6月~2017年6月间B超诊断为剖宫产瘢痕妊娠、治疗及随访资料完整的患者58例,按随机数字表法分为聚桂醇组(n=28)和甲氨蝶呤(MTX)组(n=30)两组进行对比分析,其中聚桂醇组给予超声引导下聚桂醇剖宫产瘢痕妊娠病灶局部注射联合清宫术治疗,甲氨蝶呤组给予MTX肌肉注射联合清宫术治疗。对两组患者术后1天血β-绒毛膜促性腺激素(β-HCG)水平进行比较。结果聚桂醇组术后1天血β-HCG水平与甲氨蝶呤组比较,差异有统计学意义(P<0.05)。聚桂醇组术后住院观察时间显著短于甲氨蝶呤组,差异有统计学意义(P<0.05)。结论超声引导下聚桂醇剖宫产瘢痕妊娠处局部注射联合清宫术治疗剖宫产瘢痕妊娠疗效显著,值得临床推广应用。Objective To evaluate the clinical efficacy of B guided ultrasound guided local injection combined with curettage in the treatment of cesarean scar pregnancy.Methods58cases of cesarean scar pregnancy diagnosed by B-ultrasound from June2016to June2017in our hospital were selected and divided into two groups according to random number method:polycinnamol group(n=28)and methotrexate group(n=30).Polycinnamol group was treated with ultrasound guided local injection of polycinnamol caesarean scar pregnancy focus combined with uterine clearing,and methotrexate group was treated with MTX intramuscular injection combined with uterine clearing.The serum levels ofβ-chorionic gonadotropin(β-HCG)were compared between the two groups one day after operation.Results Compared with methotrexate group,the serumβHCG level in polycinnamol group was significantly higher than that in methotrexate group(P<0.05).The observation time of polycinnamol group was significantly shorter than that of methotrexate group(P<0.05).Conclusion Ultrasound-guided local injection of polycinnamol in cesarean scar pregnancy combined with curettage is effective in the treatment of cesarean scar pregnancy,which is worth popularizing in clinic.
