重度神经损害的特发性面瘫临床与神经电生理分析

摘　　要：目的分析重度神经损害的特发性面瘫临床表现,同时探讨神经电生理检查在疾病诊断与疗效评估方面的应用价值。方法将2015年2月1日~2017年3月1日在本院接受治疗的90例特发性面瘫患者通过电脑随机编码的方式分成三组,各30例,分别是西药组、中药组以及综合组。分析总结90例患者的肌电图检查结果,对比健侧与患侧的神经电生理检查结果以及三组患者治疗半个月后的神经电生理检查结果。结果 90例患者的病程长短不同,对应的肌电图检查结果也不同,在神经电生理检查结果方面,患侧的潜伏期(3.98±0.29)ms更长,M波波幅(0.55±0.32)mV更低(P<0.05),综合组治疗后的潜伏期(5.02±1.14)ms、M波波幅(8.13±3.15)mV、MCV(51.11±5.59)m/s与正常值最接近(P>0.05)。结论神经电生理检查在特发性面瘫疗效评估中的应用价值较高,采取中西药联合针灸、艾灸等综合疗法为特发性面瘫患者治疗比较有效。Objective To analyze the clinical manifestations of severe facial paralysis with severe nerve damage,at the same time,to discuss the application value of electrophysiological examination in the diagnosis of disease and the evaluation of curative effect.Methods Ninety patients with idiopathic facial paralysis treated in our hospital from February1,2015to March1,2017were randomly divided into three groups(30cases/group)by computerized coding method:western medicine group,traditional Chinese medicine group and comprehensive group.The electromyographic findings of90patients were analyzed and compared.The electrophysiological findings of the healthy side and the affected side were compared,and the electrophysiological findings of three groups after half a month treatment were compared.Results The course length of the90patients was different,and the results of the electromyography were different.In the results of electrophysiological examination,the latent period of the affected side(3.98±0.29)ms was longer,and the amplitude of M wave(0.55±0.32)mV was lower(P<0.05).The incubation period(5.02±1.14)ms,M wave amplitude(8.13±3.15)mV and MCV(51.11±5.59)m/s were the closest to the normal value after the treatment(P>0.05).Conclusion Nerve electrophysiological examination is of high value in the evaluation of curative effect of idiopathic facial paralysis.Taking western medicine combined with acupuncture and moxibustion is effective for the treatment of idiopathic facial paralysis.
