缺血性脑卒中早期神经功能恶化的危险因素分析

摘　　要：目的探讨缺血性脑卒中早期神经功能恶化(early neurological deterioration,END)的危险因素。方法根据是否发生END将110例缺血性脑卒中患者分为END组和非END组各55例;对缺血性脑卒中患者早期神经功能恶化的影响因素进行单因素分析,对早期神经功能恶化的危险因素进行多因素Logistic回归分析。结果 END组和非END组的入院时收缩压、入院时舒张压、心房颤动、颈动脉狭窄、白细胞计数、C-反应蛋白、D-二聚体、凝血酶原时间和基线NIHSS评分等因素差异有统计学意义(P<0.05);多因素Logistic回归分析显示,入院时收缩压、心房颤动、基线NIHSS评分是缺血性脑卒中患者发生END的独立危险因素(P<0.05)。结论入院时收缩压、心房颤动、基线NIHSS评分是缺血性脑卒中患者早期神经功能恶化的独立危险因素。Objective To identify the risk factors that are associated with early neurological deterioration(END)in ischemic stroke.Method Firstly,110patients with ischemic stroke were subjected to a diagnosis for END and then divided into two groups,namely,END group(n=55,consisting of those who had END),and non-END group(n=55,including those who had no END).The factors that contribute to early neurological deterioration of ischemic stroke patients were analyzed with univariate analysis.Multivariate logistic regression analysis was exercised on the risk factors of early neurological deterioration.Findings Differences between the two groups in systolic blood pressure at admission,diastolic blood pressure at admission,atrial fibrillation,carotid artery stenosis,white blood cell count,C-reactive protein,D-dimer,prothrombin time and baseline NIHSS score were observed.The differences were statistically significant(P<0.05).Systolic blood pressure at admission,atrial fibrillation and baseline NIHSS score were independent risk factors for early neurological deterioration in patients with ischemic stroke(P<0.05).Conclusion Systolic blood pressure at admission,atrial fibrillation and baseline NIHSS score were independent risk factors for early neurological deterioration in patients with ischemic stroke.
