成人急性肺炎支原体感染引起CAP患者并发心肌损伤及其影像学特征

摘　　要：目的分析成人急性肺炎支原体感染引起社区获得性肺炎(CAP)患者并发心肌损伤情况及影像学特征。方法随机选取北京市顺义区医院诊治的CAP患者280例,采用支原体血清抗体及聚合酶链反应确诊成人急性肺炎支原体感染,其中非支原体感染组155例,支原体感染组125例,比较两组患者的一般资料、生化检验以及影像学特征。结果两组患者的性别、年龄、发热时间、体温升高程度、咳嗽、气促、肺部干湿啰音及基础病变等比较,差异均无统计学意义(P>0.05);支原体感染组的cTnT水平增高者比例显著高于非支原体感染组(P<0.05);胸部影像学结果显示,支原体感染组表现为双肺弥漫性病变及纵隔淋巴结肿大的比例均明显高于非支原体感染组(P<0.05)。结论成人急性肺炎支原体感染CAP患者常可伴随心肌损伤,胸部影像学以双肺弥漫性病变及纵隔淋巴结肿大为主要特征。Objective To analyze the complicating myocardial damage situation and imaging characteristics in the patients with community acquired pneumonia(CAP)caused by adult mycoplasma pneumoniae infection.Methods Two hundred and eighty patients with CAP treated in Shunyi District Hospital were randomly selected and definitely diagnosed by adopting mycoplasma pneumoniae serum antibody and PCR,including 155 cases in the non-mycoplasma infection group and 125 cases in the mycoplasma infection group.Then the general data,biochemical test and imaging characteristics were compared between the two groups.Results In the comparison of the gender,age,duration of fever,degree of temperature rise,cough,shortness of breath,pulmonary rales and basic pathological lesions between the two groups had no statistically significant differences(P>0.05);the promotion of the patients with cTnT level increase in the mycoplasma infection group was significantly higher than that in non-mycoplasma infection group(P<0.05);the chest imaging results showed that the proportions of double lungs diffused lesions and mediastinal lymph node enlargement in the mycoplasma infection group were significantly higher than those in the non-mycoplasma infection group(P<0.05).Conclusion The adult patients with CAP caused by acute mycoplasma pneumoniae infection are often accompanied with the myocardial injury,which are mainly characterized by double lung diffuse lesions and mediastinal lymph node enlargement.
