摘　　要：目的探讨长链非编码RNA(lncRNA)在原发性肝癌(HCC)患者组织和血液检测中的临床价值。方法选取2016年1-12月上海市杨浦区市东医院的HCC患者癌旁标本、肿瘤组织标本、血液标本、肝癌疑似患者穿刺标本和健康体检者血液标本进行研究,在异常高表达(平均变化倍数大于5)的338条转录本中挑出6条lncRNA,用实时定量聚合酶链反应和原位杂交等方法检测lncRNA AK057037在肝癌组织和血清中相对表达量,对结果进行统计分析。结果 6条lncRNA在肝癌组织和血液中呈高表达,差异有统计学意义(P<0.05)。与健康人相比,AK057037在肝癌组织和血液中的相对表达量明显增高,且在肿瘤和癌旁组织中的相对表达量差异有统计学意义(P<0.05)。随着肿瘤的进展,AK057037相对表达量有进一步提升,特别在晚期肝癌中相对表达量更高,差异有统计学意义(P<0.05)。受试者工作特征曲线结果显示,血液标本中AK057037的曲线下面积为0.834(95%CI:0.705~0.918),诊断效率优于肝癌检测常用的核酸标志物miR-29a。结论lncRNA AK057037在肝癌组织和血液中水平明显升高,可用于肝癌的临床检测。Objective To investigate the clinical value of tissue and blood long noncoding RNA(lncRNA)detection in the patients with hepatocellular cancer(HCC).Methods The HCC tissue sample,blood sample,suspected HCC puncture sample and blood sample from the persons undergoing healthy physical examination were selected for conducting the study.The six lncRNAs which were more than5-fold expression amount were picked out in the338transcripts with abnormal high expression.The expression amount of lncRNA AK057037in HCC tissue and serm tissue was detected by using the real time quantitative polymerase chain reaction and in situ hybridization.Then the results were statistically analyzed.Results The six lncRNAs were highly expressed in the tissues and blood of the liver cancer,and the difference was statistically significant(P<0.05).Compared with healthy people,the relative expression of AK057037in the tissues and blood of the liver cancer increased significantly,and the relative expression level in tumor and paracancerous tissues was statistically significant(P<0.05).As the tumor progresses,the relative expression of AK057037was further enhanced.There was a higher relative expression in advanced liver cancer especially,and the difference was statistically significant(P<0.05).The results of the receiver operating characteristic curve showed that the area under curve of AK057037was0.834(95%CI0.705-0.918)in the blood specimen,and its diagnostic efficiency was better than that of commonly used nucleic acid marker miR-29a.Conclusion lncRNA AK057037is highly expressed in HCC sample and blood,and can be used for clinical detection of HCC.
