闭合性肝损伤患者手术治疗的危险因素临床分析

摘　　要：目的 探讨手术治疗闭合性肝损伤的危险因素。方法以2010年5月至2016年5月于我院进行治疗的60例闭合性肝损伤患者为研究对象，将其随机分为手术组和非手术组，每组30例患者，回顾性分析研究其临床资料，采用单因素分析法和多因素Logistic回归分析统计分析2组患者的数据资料。结果单因素分析结果显示，血清钾(K+)、血红蛋白(Hb)、收缩期血压(SBP)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、血细胞比容(HCT)、腹膜刺激征、合并腹腔其他器官损伤与闭合性肝损伤手术治疗具有显著的相关性。多因素Logistic回归分析结果显示，SBP<96.2mmhg、血清K+<3.6mmol/L、腹膜刺激征、合并腹腔其他器官损伤是闭合性肝损伤手术治疗的独立危险因素。结论SBP<96.2mmHg、血清K+<3.6mmo1/L、腹膜刺激征、合并腹腔其他器官损伤为闭合性肝损伤手术治疗的高危因素。Objective To investigate the risk factors of surgical treatment of closed liver injury.Methods Put the60cases blunt liver injury patients from May2010to May2016in our hospital for treatment as the research object,randomly divided them into operation group and non operation group,30patients in each group,retrospective analyzed the clinical data,used single factor analysis method and Logistic multivariate regression analysis to statistic and analyze the data of two groups.Results Univariate analysis showed that Serum potassium(K+),hemoglobin(Hb),systolic blood pressure(SBP),alanine aminotransferase(ALT)and aspartate aminotransferase(AST),hematocrit(HCT),peritoneal irritation,with abdominal significantly associated with other organ damage and treatment were closed with hepatic trauma surgery.Multivariate Logistic regression analysis showed that SB P<96.2mmhg,serum K+<3.6mmol/L,peritoneal irritation sign,combined with other organ injury in abdominal cavity were independent risk factors for surgical treatment of closed liver injury.Conclusion High risk factors for surgical treatment of closed liver injury were SB P<96.2mmHg,serum K+<3.6mmo1/L,peritoneal irritation,and other organ damage in the abdominal cavity.
