结核分枝杆菌L型血流感染与肺癌患者红细胞免疫功能的相关性分析

摘　　要：目的研究结核分枝杆菌L型(TBL)血流感染与肺癌患者红细胞免疫功能改变存在的关联。方法回顾性选取肺癌患者50例(其中TBL阳性者28例,阴性者22例),肺结核病人18例,健康人40例为对照组。采用溶血离心培养法和滴片法结合免疫酶染色检测结核患者血液中的分枝杆菌L型,采用常规方法测定并比较三组患者血清红细胞C3b受体花环促进率(RFER)、抑制率(RFIR)、红细胞沉降率(ESR)、免疫复合物花环率(ICRR)和红细胞C3b受体花环率(C3bRR)。结果肺癌组、肺结核组较对照组的RFER均明显降低,但肺癌组与肺结核组比较差异无统计学意义(P>0.05);三组之间RFIR互相比较,差异无统计学意义(P>0.05);TBL阳性组的ESR比TBL阴性组明显升高,差异有统计学意义(P<0.05);与TBL阴性组相比,TBL阳性组的ICRR和C3bRR显著偏低,差异有统计学意义(P<0.05);而两组的RFER均较对照组明显降低,差异有统计学意义(P<0.05)。结论结核分枝杆菌L型血流感染与肺癌患者红细胞免疫功能存在关联。Objective To study the relationship between mycobacteria L-forms Infection and C3bRR、RFIR、ESR、ICRR in red-cell of patients with Lung cancer.Methods50cases of lung cancer(TBL positive28cases,TBL negative22cases),18cases of tuberculosis and40healthy persons were selected as the control group.The hemolysis centrifugal culture method and drop staining of Mycobacterium tuberculosis in L blood detection combined with immune enzyme were determined and serum erythrocyte C3b receptor rosette rate(RFER)the inhibition rate(RFIR),erythrocyte sedimentation rate(ESR)and immune complex rosette(ICRR)and erythrocyte C3b receptor rosette rate(C3bRR)were compare by conventional methods.Results RFER of lung cancer group and pulmonary tuberculosis group were significantly lower than that of control group,but there was no significant difference between lung cancer group and pulmonary tuberculosis group.There was no significant difference in the RFIR of red blood cells among the three groups.The ESR of TBL-positive group was significantly higher than that of TBL-negative group which the difference was statistically significant.Compared with TBL negative group,ICRR and C3bRR of TBL positive group were significantly lower than those of TBL negative group(P<0.05).RFER in both groups was significantly lower than the control group,the difference was statistically significant(P<0.05).Conclusion There is an association between mycobacteria L-forms Infection and red blood cell immune function in patients with lung cancer.
