多普勒能量图判断颈动脉粥样硬化斑块形成前后管腔内血流状态变化的价值

摘　　要：目的评价多普勒能量图判断颈动脉粥样硬化斑块形成前后管腔内血流状态变化的价值。方法采用回顾性研究方式选择120例行颈动脉超声检查的颈动脉粥样硬化患者作为研究对象,根据颈动脉粥样硬化是否存在斑块分为两组,63例无斑块者设为对照组,57例有斑块者设为观察组。采用多普勒能量图观测两组患者的颈动脉血管边缘区域血流变化情况,对比两组患者左侧、右侧的颈动脉中-内膜厚度和能量多普勒波型分型。结果两组患者的性别、年龄、糖尿病、高血压、吸烟史、家族病史、总胆固醇、甘油三酯、低密度胆固醇、高密度胆固醇等一般资料方面的差异均无统计学意义(P>0.05);观察组左侧、右侧的颈动脉中-内膜厚度(IMT)均大于对照组(P<0.05);观察组能量多普勒波型的失衡波型比率高于对照组(P<0.05),均衡波型比率低于对照组(P<0.05)。结论多普勒能量图可以较为有效地判断颈动脉粥样硬化斑块形成前后管腔内血流状态变化,为判断颈动脉粥样硬化斑块形成或进展风险提供有力的影像学参考。Objective To evaluate the significance of Doppler energy figure judgment for changes in status of blood flow before and after the formation of tube cavity by carotid atherosclerotic plaques.Methods A retrospective study on 120 patients with atherosclerosis of carotid artery with ultrasonography as study objects,they were divided into 2 groups according to the presence of carotid atherosclerotic plaques,63 cases without plaque were set as control group,and 57 cases with plaque were allocated in observation group.The changes in blood flow in carotid artery border area of patients in these two groups were observed by Doppler energy imaging.The left and right carotid artery-lining thickness and classification for energy Doppler wave forms had been compared betwewen these 2 groups of patients.Results The difference in gender,age,diabetes,high blood pressure,smoking history,family history,total levels of cholesterol,triglyceride,low density cholesterol,HDL cholesterol and other general informations between these 2 groups of patients had no statistical significance(P>0.05).The intimal thickness(IMT)of left and right cartid arteries in patients of observation goup was greater than that of patients in control group(P<0.05).The ratio of energy imbalance Doppler wave types in observation group was higher than that of control group(P<0.05).The equilibrium mode was lower than that of control group(P<0.05).Conclusion Doppler energy figure can be more effectively judged for the changes in blood flow before and after the formation of tube cavity by carotid atherosclerotic plaques,hence it can supply the risk of atherosclerotic plaque formation in carotid arteries or provide powerful imaging reference.
