卡介菌多糖核酸联合玉屏风颗粒治疗慢性荨麻疹的疗效及机制分析

摘　　要：目的探讨卡介菌多糖核酸联合玉屏风颗粒治疗慢性荨麻疹的疗效及对血清中白细胞介素-2(IL-2)和白细胞介素-6(IL-6)的影响。方法将慢性荨麻疹患者90例采用随机数字表法随机分为治疗组和对照组,各45例。对照组口服地氯雷他定片,1片/次,1次/天;卡介菌多糖核酸注射液,肌内注射,每次1 m L,2~3次/周。治疗组采取卡介菌多糖核酸注射液,肌内注射,每次1 m L,2~3次/周;玉屏风颗粒,开水冲服,每次5 g,3次/天。两组患者连续治疗8周。比较两组患者的症状评分、临床疗效及复发率。检测两组血清IL-2及IL-6水平。结果治疗组治疗后患者的瘙痒程度、风团数目、风团持续时间、每日风团发作次数及风团最大直径评分显著低于对照组同期(P<0.01)。治疗组患者的愈显率为91.11%,显著高于对照组为71.11%(P<0.05)。治疗组复发率13.33%,明显低于对照组复发率33.33%(P<0.05)。治疗组治疗后患者血清中IL-2及IL-6水平明显高于对照组同期,差异有统计学意义(P<0.01)。结论卡介菌多糖核酸联合玉屏风颗粒治疗慢性荨麻疹的疗效明显,上调患者血清中IL-2及IL-6水平可能是其作用机制之一。Objective To explore efficacy of polysacchafide nucleic acid fraction of bacillus Calmette-Guerin(BCG-PSN)combined with Yupingfeng granules in treating chronic urticaria and the influence on serum levels of interleukin-2(IL-2)and interleukin-6(IL-6).Methods Ninety patients with chronic urticaria were randomly divided into control group(45cases)and treatment group(45cases).Patients in control group were given with Desloratadine tablets(1time/d and1tablet/time)and BCG-PSN injection(intramuscular injection,1mL/time,and2-3time/week).Treatment group was treated with BCG-PSN injection(intramuscular injection,1mL/time,and2-3time/week)and Yupingfeng granules(boiling water,5g/time,and tid).Two groups were treated for8weeks.Scores of symptoms,efficacy and recurrence rate were compared between two groups.Serum levels of IL-2and IL-6were detected in two groups.Result After treatment,scores of itch degree,numbers,lastin,daily time,and sizes were evidently lower than those of control group(P<0.01).Cure rate of treatment group was91.11%,which was remarkably higher than control group71.11%(P<0.05).Recurrence rate of treatment group was13.33%,which was lower than control group33.33%(P<0.05).After treatment,serum levels of IL-2and IL-6in treatment group were obviously higher than control group with statistical difference(P<0.01).Conclusion Efficacy of polysacchafide nucleic acid fraction of bacillus Calmette-Guerin combined with Yupingfeng granules in treating chronic urticaria is evidently significant,and may be relevant to up-regulation of IL-2and IL-6.
