摘　　要：目的对比分析老年女性糖尿病患者初发急性心肌梗死的临床表现及冠状动脉造影(CTA)特点。方法将临床确诊的41例老年女性糖尿病初发急性心肌梗死患者作为A组,41例不伴心肌梗死的老年糖尿病患者作为B组。两组患者均行CTA。比较两组临床资料、心电图特点,冠脉病变支数及狭窄程度等。结果与B组相比,A组患者年龄大、Hb A1c水平高、糖尿病病程长、TG水平高、LDL-C水平低、冠状动脉病变狭窄程度重(P<0.05)。A组患者ST段抬高型心肌梗死较少(31.7%),梗死部位多为下壁(46.3%)。结论初发心肌梗死糖尿病老年女性患者临床表现有一定特征,对于血糖控制较差病例应及早预防。Objective To comparatively analyze the clinicl features and imaging changes of CTA of primary acute myocardial infarction in elder female diabetic patients.Methods Forty-one elder female diabetic patients with primary acute myocardial infarction were selected as the group A and 41 elder diabetic without acute myocardial infarction were selected as the group B.All patients underwent CTA.The clinical data,characteristics of ECG,number of diseased coronary artery and degree of coronary artery stenosis were compared between the two groups.Results Compared with the group B,the patients in the group A were older,higher in HbA1c,longer in the course of diabetes,higher in TG,lower in LDL-C,and more serious coronary artery stenosis(P<0.05).In the group A,the patients with ST segment elevation myocardial infarction were less(31.71%),and the infarct sites were mostly the inferior wall(46.34%).Conclusion The clinical manifestations of early onset myocardial infarction in elderly women with diabetes have certain characteristics,and early prevention of poor blood glucose control cases should be taken as early as possible.
