摘　　要：目的研究桥接外固定架结合负压封闭引流技术在胫腓骨GustiloⅢ型骨折治疗中的应用效果。方法选择2015年12月-2016年12月我院收治的58例GustiloⅢ型骨折患者进行研究。按随机数表法,随机分为观察组和对照组,每组各29例患者。对照组患者采用桥接外固定支架结合传统创面换药的治疗方式;观察组患者则采用桥接外固定架结合负压封闭引流技术治疗方式。观察并记录患者术后并发症情况、首次进行功能锻炼时间、骨折愈合时间、创面愈合时间;对出院患者进行康复随访,观察患者康复情况并进行综合评价。结果术后,观察组患者首次进行功能锻炼时间、骨折愈合时间、创面愈合时间明显短于对照组(P<0.05);骨髓炎、伤口感染、关节肿痛等并发症总发生率明显低于对照组(P<0.05);观察组患者术后康复情况综合评分明显高于对照组(P<0.05)。结论采用桥接外固定架结合负压封闭引流技术治疗,一方面能有效降低并发症发生率,减轻患者身体负担;另一方面增强患者的康复效果,加快患者的康复速度,值得广泛推广应用。Objective To study the effect of bridge external fixator combined with negative pressure closed drainage in the treatment of GustiloⅢtype fracture of tibia and fibula.Methods 58 patients with type GustiloⅢfracture treated in our hospital from December 2015 to December 2016 were studied.According to the random number table method,they were randomly divided into observation group and control group,29 patients in each group.The control group was treated with bridging external fixator combined with traditional wound dressing,while the observation group was treated with bridge external fixator combined with vacuum sealing drainage.The postoperative complications,functional exercise time,fracture healing time,wound healing time,follow-up rehabilitation for discharged patients were observed and recorded,the rehabilitation of patients and make comprehensive evaluation were observed.Results After operation,the patients of the observation group in the first functional exercise time,fracture healing time,wound healing time were significantly shorter than the control group(P<0.05);Osteomyelitis,wound infection,joint pain and other complications were significantly lower than the control group(P<0.05);The patients of the observation group after rehabilitation score was significantly higher than the control group(P<0.05).Conclusion The treatment of bridge external fixator combined with vacuum sealing drainage technology can effectively reduce the incidence of complications and reduce the physical burden of patients.On the other hand,it will enhance the rehabilitation effect of patients and speed up the rehabilitation of patients,so it is worthy of wide application.
